There was one on 24. In one scene in season 2, Tony Almeida (Carlos Bernard) is seen using a cane at work. It was due to a basketball injury, but in the show, it was explained that it happened when Jack Bauer (Keifer Sutherland) knocked him down in the hallway when Tony tried to prevent Jack from leaving.

I'm really shocked Walking Dead isn't on here. Norman had a really bad concussion for the final episodes and they wrote in that he got hurt because of the bruising on his face. Like it was really bad. He was in the hospital.
