This way of thinking might seem unrealistic/utopian to YOU but it is very real for billions of people around the world. Imagine calling a certain religions perspective ridiculous and unrealistic. The doctor would not ask about drinking or sleep before diagnosis because they would have no reason to know or think its a factor in what the current situation is. Once you get diagnosed and placed in the facility it no longer matters what you say because they look for cooperation and behavior to let you out. Its all a pretty quick process from diagnosis to placement in facility. Afterwards they would no longer care about substances or sleep deprivation.
