The two arm holes, the head hole, and (in my experience at least) 2 button holes. The hole for the body isn't actually a hole; it's the edge of the disc you could flatten the shirt into if the material were cooperative enough

Polos have the word ‘polo’ on them so that might be why. Also so could argue that the single whole counts as two wholes.
It's because it's how many holes in "a polo" a, p, o, o have holes. No overthinking required, it's the same as "the alphabet" question
