Leadership lessons: Creating an environment where it is safe to speak up

In 1628, a massive Swedish warship named the Vasa set sail on its maiden voyage. But before it left the Stockholm harbor a light gust of wind blew its side, and the magnificent warship keeled over. Why did the Vasa sink?
We propose that that is important to create an environment where it is safe to speak up, otherwise, people would rather let you sink the ship than speak up.
