Its best to go first class on these trains if you have the option and can afford paying a bit extra. I found it helpful in some countries to just pay for the upgrade, the bathrooms are much better in first class, and with the conversion rate depending on where you come from; the upgrade ticket is like paying economy somewhere else.

I do feel bad for those who dont have enough money and have to travel in such unhygienic conditions but after all its these people and the authorities responsible for this, so if you ever visit any country in south asia make sure you dont act cheap and experience the conditions poor people do here unless you want to ofcourse.

If you want top-of-the-line, airplane-level cleanliness, brother, just book first class. If you don't have the money, you can always book an H2, which is in between full low-tier class and first class. It's money-efficient and cleaner.
