Mark Margolis, playing Hector, is a superb actor. I've admired his work since I first saw him as Sol in Darren Aronofsky's debut film Pi. In which, come to think of it, his character also suffered a stroke. The venom & utter hatred Margolis imbues Hector with is outstanding.
As an occupational therapist myself, it's amazing to see one mentioned here in this case. They mentioned one in Hank's recovery as well, although the rehabilitation process of patients with disabilities acquired from bullet injuries are different from those acquired from a stroke (since neurological conditions are far more complex, anatomically speaking). I have to say, even if it seems fast paced, Hector's recovey, coming from a coma state, to the point where they were able to establish an alternative communication method (his index finger movement), seems pretty realistic to me. It's a pity that Gus had to interrupt the rehabilitation process (cause of his revenge-related goals), because studies show electrical stimuli theraphy is shown to be very effective in stroke cases. Of course, there's no way of telling that Hector would have been able to walk and be completely functional again (he's still an elder, despite being a cartel boss), but considering the medical response time, he might have been a better prognostic than just a wheelchair with just one finger movement preserved.

I honestly wish more actors on the show spoke Spanish. You can see that the dialogue oddly switches back to english (between a whole group of ostensible Spanish native speakers) whenever Gus or Hector are in the scene, because those two don't speak Spanish. It takes massively away from the authenticity of the scene.

She breaks the ice going to Spanish, never waivers, using language they can understand, keeping momentum trying to introduce a compassionate scenario to their world. At the conclusion touches Marco on the arm which seems to be bridging a huge huge barrier that really no one dares to do. The twins seemed totally bemused yet still seem unable to show empathy in a situation they cannot control.
Before i got on as a Firefighter at age 19 i was a janitor/custodian in a hospital and this is funny how the staff pander and suck up too another expert technician from a prestigious hospital. I did the hospital job for about 18 months and i think a grand total of a dozen people who worked there ever even said a hello to me. Once in a while you did see these criminal cartel types in there and when they passed me they always said hello or at least nodded
