Hi Marcia, Of course you can use Orficast to make orthoses for the thumb, index long and ring fingers!! This particular design is for a PIP dynamic extension orthosis- you might consider placing the MCP joint in flexion to make this orthosis more effective on the PIP joint flexion contracture. This is not an orthosis for stretching the lumbricals!!!! To stretch the lumbricals, the MCP joints need to be in slight hyper-extension and your client should work on the active and passive hook position. The correct orthosis would hold the MCP joints in extension or hyper-extension and pull the fingers into full PIP and DIP flexion.
