While standing with the superintendent of an owner/operator company, who's workers were doing construction work on their site, we watched several men climb into one then use it as an elevator to a cooling tower. Problems were: 1. The Men weren't wearing harnesses/tied off inside the unit. 2. We observed the men climb over the hand rail and over the guardrail on the tower without being tied off. 3. The tower was only as high as a two story building roof, and there was a stairway from the ground to where they were going. (Not inaccessible). 4. When I asked to see the operator training certification, I was informed that he didn't have one. In fact he had just been hired because he told the superintendent, he knew how to operate one. 4 violations with multiple exposures.
