Here's my take - first verse he's setting the stage, he's saying very contradicting back and forth things (I think your Onstar button is stuck, out of gas, I'd hate for you to be stuck) then proceeds to tell us what he does taking advantage of the situation, NOW you can relate the mindset to the album title RELAPSE and apply the drugs (he's up and down off drugs) and in and out of being level headed. 1st victim is wheres he's hooked on the drugs and is dependent on them (he's relating the what the drugs are doing to him in comparison to "relationships" and mentions party, fun, rehab) things are happening in the beginning of of the drug addiction phase, including "sick stunts" slipping up and not having full control of your actions and people are noticing them (in and out of rehab) then he says "let's go to rehab" and then follows up by saying "let's have an intervention" which means the relapse phase is about to reach over dosing as he says "murder wasn't my intention" had to much fun and could have died. You won't. 2nd victim was even bigger, so he's relating to increasing the dose even higher during the Relapse, then says how (the whole world was hers) so this is the peak of addiction and using, he's getting mad at the (mega buzz) that she's getting from fame (but he's in the drug mega buzz state) then transitions to using the word "Crush" as in he wants the level she's on, and also which would be crushing the meds and deepening the dependency/addiction, when he starts explaining how he feels "cut off an ear, collections of skeletons, impaling himself with an elephant tusk (he's not trying to hear any cries for help, tons of empty pill bottles aka skeletons, consuming a large amount of ivory/white probably thru the nose or needle). Then he decides this is the night as he sets the visual stage, has Seroquel for the psych/bipolar moods, then asks "will you share a pill or two with me" he wants to be on the same level/mindstate as her as he proceeds to say "I'll trade you a blue one for a pink one" could be making a reference to kids trading candy as he says "ever since as a schoolgirl" but again just wanting all the different types of drugs to try and cope with the crazy level of fame they are both sharing. So it's the same song and dance (which is normally portrayed as just a fun activity/situation, which at their level is overwhelming and hard to deal with that level of fame while sober) thus leading to a breakdown and or overdose and leading to a psychotic state where (portrays in the album single 3 A.M.) the dark and twisted nature that became from the fame and drugs!
