I've had it happen too. Seems like there is a very small cast time on fade. If you are hooked during that cast time it will counts as a hook. It's such close timing that out of the 100's of hooks I've dodged 3 actually caught me. Same with ults that 1 tap. If I fade at the exact same time but im off by a quarter of a second I'll die anyway even though I swear on my life I fade at the right time.
