  • Be sure to watch "How to do an ABI" part 2 to complete the video. This educational video teaches how to correctly perform the Ankle Brachial Index test while citing statistical facts about Peripheral Arterial Disease (PAD) in the United States. The ABI is a simple to do test comparing the patients Ankle Systolic blood pressure to their Brachial Systolic blood pressure. The ratio or index should be 1.00 - 1.29.

    If patient is standing up or sitting, the pressure can be significantly higher on the legs, therefore the results would not be accurate. The test has to be done lying down. You want a 1.0 ratio. 1.0 ~ 1.3 normal, It is easy to do the test the wrong way and get results way above the 1.3 threshold and you get false results. I think some people panic when they workout or get leg pain and think they have PAD, when there are similar symptoms for other conditions that are not related. :)

    To keep it simple, all pressures should be taken at the phlebostatic axis (level with the heart)

    The Doppler should have the tip of the probe pointing towards the patient's heart (which is towards the head for arms and legs) because arterial blood flows from the heart towards the arms and legs. The Doppler senses the blood flow coming towards it.

    Hopefully by now you have the answer, but here you go anyways. The pt needs to be in supine in order to avoid the orthostatic pressure that comes from being upright. This ties directly to your second question which again, if you are raised up, your blood pressure is falsley elevated due to the sheer weight of the blood. When the pt is in a flat position the weight of the blood is eliminated and the pressures (brachial and ankle) ,at least in theory, should be unadultarated and equal.

      The patient should be allowed to rest for at least 10 minutes, and the extremities
      should be comfortably warm during the measurements.

      The clinician authorized to perform and interpret the ABI tests varies by region and country. In the US, vascular technologists perform the tests and physicians interpret the results.
