I am sorry, I am unable to remember the exact model, however, if you measure the header diameter and how far back you need to go you can narrow your search. Be advised you will need to custom fabricate a mounting bracket, I used 1/8 inch stock.
Yes, I also added K&N Pods but cant remember the jet sizes i used. I went to jets r us and if I had to do it all over again i would have just ordered a few sizes to save me from reordering.
