I think the ship is fitted with holographic projectors throughout the ship, so the doctor only uses his emitter on away missions. Fun episode.

They have a strange relationship with Janeway because she is technically a stranger. And they aren't used to adults they can trust, they were all slaves, some of which were sold by their parents. They are used to anything slightly outside of the rules needing to be kept hidden.
