    Does this block cover the hand bones too , or do you need to block the deep radial nerve for this??

    What should I do when applying tourniquet to a patient?

  • @musalone1
    @musalone1 3 ปีที่แล้ว +1

    Great video ,thank you so much. Just wanted to know does your surgeon need a tourniquet for the CTD if does where you put it. Mid forearm TQT may not be comfortable though the procedure is a quick one. My surgeon always needs an arm tourniquet and he does the block himself at the crease of the wrist…

    Yo lo que quisiera saber si con las inyección no lastiman el nervio del brazo mi pregunta es porque yo fui operada de la planta facial con la técnica tenex y ya tenía el nervio tibial dañado y con la operación se empeoró ahora no solo me duele al caminar si no que me lástima al estar parada y yo sé que el dr lo inyectó porque el agujero de la Abuja tardo en cerrar me operaron en diciembre 22 y asta el día de hoy está inflamado ahí yo ya lo tenía dañado y ahora me duele el doble

    At 14:22 minutes after start of the video, it is mentioned that interosseous artery accompanies median nerve. Is this true ?

    I have had 3 Beir blocks, they worked great and lasted a long time. Very simple procedure , just curious as to the benefits of this procedure vs a Beir, which seems a lot easier with less potential of complications. Anyone? Thanks in advance, binge watching these!

    • @musalone1
      @musalone1 3 ปีที่แล้ว

      B block definitely so much simpler for this 10 minutes procedure….I can’t disagree with you…you need US and special needles….where as BBlock all I need is a double lumen Cuff…my surgeon has asked for B Block for revision CTD…MPH, Taunton.

      Bier block is an obsolete 100 year old block. If you don't want to do a real nerve block just give propofol.

    • @rougerush

      I find Bier block is excellent for shorter procedures of the hand as it is limited by tourniquet use, patients Will develop pain,so it depends on the surgeon expertise and I also find limited use in fractures due to manipulation to accomplish exsanguination 😊

    Is it safe to associate a vasoconstrictor (epinephrine) to the local anesthesic in these forearm blocks?

    is it possible to combine a walant and trunk block for carpal tunnel surgery and finger surgery.
    Thank you 🙏

    • @DRBLUESNYC
      @DRBLUESNYC 3 ปีที่แล้ว +2

      By all means. Walant technique is an infiltration of local anesthetic. There are 3 drawbacks to Walant: 1) You need time to do this properly and supplement as necessary 2) It causes swelling of the surgical field 3) It does not provide anesthesia for Tourniquet. So, as long as you have enough time (not too many patients), do not mind swelling and you can operate fast so that Tourniquet is not an issue, it is sufficient. Our surgeons value our service and wrist blocks because they do large volume hand surgery - one surgeon does as many as 10-30 patients/day. The only way you can do this - is with a dedicated regional anesthesia service. Best regards

    Hi sir , I have a question
    Some surgeons prefer an arm tourniquet for carpel tunnel release and also metacarpal procedures ,
    Then should we prefer a infraclavicular approach of brachial plexus blockade along with an infiltration in the arm to block ICB nerve for tourniquet or can we go ahead with a forearm wrist block and an infiltration of ICB nerve for tourniquet ??
