I don’t think he could exile the graveyard in response, because the active player goes to combat and holds priority at that point Ulamog was already exiled. Basically the exile ulamog happens first and then exiling the graveyard after that makes no sense at all.
Emperor targets, so you can respond to the target from the ability. The Player could exile before Combat too, which wasn't done either. Can respond to the Frog's ability as well, as Discarding is the activation clause so it's in the yard before you put a counter on Frog. I think it was just a mistake with timing that ended up costing the whole game.
If the card reads target, then that passes priority over to the other player if they want to respond. There's multiple ways he could have handled this situation depending on what he had in hand. Optimus already explained the 2 but if he also had a kill spell like discharge or Lightning bolt, it would have had the same effect. Bones player goes to target, you play kill spell, they're forced to waste mana to adapt and not let it die or let it die, card then gets exiled but it got exiled AFTER the adapt mana ability happened, thus no reanimate.
