    Nice and straight to the point,thanks. Headaches and other signs of increased icp e.g. papilledema usually are not seen.

    Thanks for the video, but you can explain how the ventricle enlargement causes Apraxia Gait, Renal Incontenance and Dementia?

      Hello, so Arachnoid granulations are not functioning as they should, and therefore the Cerebrospinal fluid can not drain from the subarachnoid space into the venous sinuses. This means that pressure builds up and the ventricles expand.
      The expanded ventricles can compress the central portion of the corona radiata. This can lead to interstitial edema + impaired blood flow to the periventricular white matter. The periventricular white matter contain sacral motor fibers that innervate the legs and bladder. So the edema + impaired blood flow --> causes damaged periventricular white matter --> that causes damaged sacral motor fibers --> that causes damaged innervation to legs (Ataxia) and damaged innervation to bladder (Incontinence).
      Ataxia can also be caused by the compression of the brainstem, for example the pedunculopontine nucleus.
      Dementia can be caused by the compression of the periventricular limbic system.

      The corona radiata gets stretched, it has motor tracts that run through it (from cortex). These tracts involve leg fibers and bladder fibers.

    why the lumboperitoneal shunt was not mention in this case? would it work the same as VP shunt if there arent obstructions?

    No mention of the specifics of the MRI or CT reading? The test should show ventricular enlargement out of proportion to sulcal atrophy.

      He did say that it’s not enough though. The walk, dementia, and urinary issues must also be present. And then the other form of ventricular? dementia will have these characteristics as well. Then the lumbar tap is done, and if symptoms improve then the drainage occurs us my understanding

    Hi doctor, my daughter (16y) had diagnosed with a hydrocephalus 3 years ago, she did a surgery (shunt) and after year it blocked and she undergo another surgery (shunt) and also after year blocked or failed, she did an endoscopic surgery and now (after 6 month from surgery) the symptoms are recurrent again 😭 what should we do

    wait wait wait. im confused dementia is progressive however once you correct NPH the forgetfulness should be eliminated, no? Also if the CSF is accumulating in the brain why not use a shunt to directly remove accumulation instead of lumber puncture?

      The lumbar puncture is a diagnostic tool. As stated in the video, the symptoms of NPH can also be present in vascular dementia. If the patient's symptoms improve after an LP, then the diagnosis of NPH can be made, however if the symptoms do not improve you have to consider other options. If the patient has improvement of symptoms with the LP, a shunt can be placed as the treatment of NPH.
