I love the name of this album. Dolls in Pseudo Paradise are exactly what Touhous are. Because ZUN and us can't actually go to paradises, all we do is imagine beautiful girls there having fun. Girls who aren't actually real. All we're doing is playing with dolls. I think that's why so many songs in this album are a bit melancholy.

It's actually quite a selfish place. It's also somewhat of a study on human nature. Something like "even in the realm of fantasy, humans are optimistic to the point of delusion or cruelty". It's just like when you read a newspaper in real life and don't care about a few deaths. ZUN could be saying that humans are the same everywhere; callous and selfish.
You forgot about the "Pseudo" word in the title. I say, in a first view, Gensokyo looks like a real paradise with fun, girls everywhere, magic, alcohol, "friendly" battles, etc. But the place is really bad for a normal human, because is in death danger every minute of his life.
This is honestly my favorite touhou song period. The main motif masterfuly captures the beauty and dager of fantasy and gow it can co exist with reality and the beliefs of the people....imo ofc. 3:25 absolutely beautiful, the absolute fantasy

My theory is that the picture represents 2 shrine maidens are from the future. Maribel and Renko are both from the future, the "CD's backstory" is related to the duo after all. In this work there are multiple mentions of a shrine maiden, who from the description and depictions doesn't seem to be Reimu. And why would Hieda no Akyuu- the chronicler of Gensokyo- be a shrine maiden?
I find it more believable that the purple-haired miko shown in the image is Akyuu for two reasons. First, I think it's more likely that Remilia would be drawn into an image with Akyuu and Reimu rather than an image of the DiPP miko and Reimu. Maybe Akyuu is just cosplaying or something. Secondly, there's a purple-haired miko featured on the first edition cd disk label, which looks completely different to the girl drawn in this image. You know what? I feel like the Touhou Project is a prime example of the differences between Canon and Fanon. The canon Touhou serves as the "spine" of the series. (That doesn't mean that it can't be good, though!) And even if it (...eventually...) ends, the fan material will still remain active. One of the neat things about the fanon side, however, is that a number of alterations can be made to it. We can't be 100% faithful all the time to any of the material, so it's fine to put changes in. As another comment here said, all were doing is playing with dolls. And everyone has their own way of playing with dolls/writing. So, at their core values, there's really not that much distinction between, say, TH6 and Luna Nights, the only thing is that TH6 was made by the creator of the canon/spine, with his own writing style.
