How is it that when you enter the opening to the ground level gates, its at ground level, but when you enter the terminal via the gates, you have to take an escalator up 1 level? Is the gate concourse a bit slanted, so one end is at ground level, while the other end is underground?

Considering how much the port authority owns and how much they charge for everything this terminal should be spotless and look well maintained.
