Cantarella is basically a story composed into a song. It's about an incestuous love although the Miku and Kaito are not blood related. They just portrayed the story. Plus, the title Cantarella is kinda related to Romeo & Juliet. There was a scene in Romeo & Juliet where Juliet drank a sleeping drug which she thought was poison, she ended up sleeping for 1 day with no detectable pulse. Yes, that drug is called Cantarella and it's a pretty tricky drug 'cause it makes you look like you're dead. Also, Cantarella is the "trap" Kaito mentioned in the song. :)
Sorry, but tetrodotoxin isn't as same as the Cantarella poison that the famous Borgia made. Cantarella poison is made from cantharidin (a powder made from blister beetles), while Tetrodotoxin is a poison that came from pufferfishes (porcupinefishes, ocean sunfishes, or triggerfishes have this also).
