Im now 72, four years ago I had cataract surgery and had the multi focal Tecnis Symfony lenses installed. My close and distance vision is now fantastic! I do not need any other vision correction (contacts or eye glasses) and have been so impressed with this procedure. If your eyes are otherwise healthy aside from the cataracts, I heartily recommend inter-ocular implants!

One week post surgery, and I'm already reading my phone without readers. Distance vision is pretty good, but not as good as my non-operative eye. The doc says both will improve as the cornea swelling continues to decrease. It doesn't feel swollen; I'm very comfortable. Driving is fine during the day, haven't tried at night yet. I haven't noticed the halos in the house, though, so that problem might be fixed. Having three different prescriptions in this lens is as natural as breathing. Very satisfied!

I am turning 67 in 5 days and am getting a Symfony lens during cataract surgery the day after. I am ambliopic so have only really only used one eye my whole life. Further...my ambliopic eye had a detached retina just before COVID started, so Fall of 2019 and is even more useless than before the retina detached and was corrected... but always so bad that the change is not that noticeable. Now I only use glasses for very close up Engineering work, but not very often and they are cheap ones for about 4 Pounds.
It was about £6000 for all 3 ops, but no more wearing glasses in my every day life, and when the dog tries to lick my face I don't get dirty glasses

I was very scared too. But the next next day after a bandage was removed I was amazed! We went to eat at a restaurant and I was in awe how clear everything was. Colors were so vibrant and details I hadn’t seen for years had returned. I couldn’t stop looking at everything in the restaurant.
I only had my left eye done and it was that dramatic. I was going to schedule to have my right lens replaced but then the Covid pandemic hit so I didn’t want to expose myself any further. But now my right eye has become remarkably cloudier so I’ve scheduled surgery to have my right lens replaced.
At the time I had my left eye done I didn’t know of the options this doctor mentions in his video. I got the lens that would give me great far vision because I’m so used to using reading glasses anyway. I’m not sure if I would’ve opted for one of the lenses in this video. But it does sound good to not have to wear glasses at all.
I wish you well! I know you’re afraid and that’s understandable. I was really scared until the operation went so well and especially after the bandage on my eye was removed and I could see dramatically better. It was like having eyes I had when I was a kid.
      All the best!

Dr. Wong, I have been nearsighted most of my life (20-80 in both eyes), and I started using soft contacts to correct far vision in 1982. I am currently 70 years old. When wearing my contacts, I needed reading glasses for good close up vision. In 2006, my left eye seemingly was rejecting it's contact, so I decided to keep it out for several days, and in the process discovered that not only did I no longer require reading glasses for close up work, but that I could still see very well long distance, and over the next few weeks virtually all "fuzziness" from up-close to distance viewing was virtually completely eliminated. An additional effect was discovered 5 years ago when I had my vision checked, and my corrected eye (with the contact) was still at 20-80, but the uncorrected eye was now at 20-40. Now, at 70, my night vision has degraded to the point to where I will be needed cataract surgery soon. My question for you is do you feel I will be able to have differently focused monofocal lenses installed in my eyes during the surgery to duplicate my single contact derived monovision? Thank you in advance for your response.

  • @StevenGSafran
    @StevenGSafran 3 ปีที่แล้ว +3

Shannon my experience exchanging these lenses has been that while the quality of vision with the Symfony is A for distance to intermediate as you demonstrate, the Panoptix quality of vision for all ranges is not as good as the Symfony or a monofocal is for distance ....more like B+ for most patients. When we have a a Symfony in one eye and a monofocal in the other eye patients feel the distance quality of vision is the same whereas if there is a Panoptix in one eye and either a monofocal or a Symfony in the other eye patients will notice a significant difference in sharpness, contrast and quality of vision in the two eyes with the Panoptix not being as good. When patients are unhappy with Symfony it is because of distracting starbursts whereas most of the patients I see on referral unhappy with the Panoptix are bothered by quality of vision issues ( and halos ) which resolve with lens exchange. In my practice for my own patients I prefer the Symfony for this reason.

Halos are not good, but I do have some cloudiness and wrinkles in my capsular bag so may get a YAG treatment in 2 months. That may help with the halos blurriness and glare I'm having; so I'm told. @dopeyrn1 Thank you for replying. I think you were wise to do the single vision and am sorry to hear that you are having an issue with that eye. I am afraid to do the YAG laser for fear of it not working, and then if you are not happy with the lens after YAG , some doctors will refuse to do a lens exchange because it's much riskier.! I've got to decide if I want to exchange for single vision or take the chance laser will work. I'm getting a second opinion before I go ahead with laser. It really never is easy and usually more complicated than we are lead to believe.

I'm a 57-year-old diabetic with cataracts in both eyes, left one significantly worse (someone once told me that taking insulin may accelerate cataract development). Being on Medicaid, I have no realistic choice of IOLs. My initial feeling is to have one close and one far monovision lenses, as my mom has in her eyes. I'm scheduled next week for a trial fitting for contacts that will simulate the effect to see if my brain adjusts properly before having the actual surgery. My dad has premium lenses in his eyes and your explanation of light transmission explains why my dad needs every light on in the house and my mom can't stand the bright light. In my initial consultation with the eye surgeon I'd never had so many different scans and photos taken. Besides cataracts I also have astigmatism. I hate wearing bi-focals - hopefully with one eye near and one far I won't need them. One thing I won't miss is the starburst effect that headlights and streetlamps have.

Your videos are amazing, Dr. Wong! Thank you for explaining the differences between the Symfony and PanOptix lenses.
My doctor is recommending the PanOptix, but I have several concerns. I am most concerned about the halos and glare at night.
    * Is the glare/halos less after the lens implant than it was with the cataracts?
    * Do you get glare/halos with monofocal lenses?
    * How does the glare/halos with the PanOptix compare to monofocal lenses?
    * I am also concerned about the scratches or scuffs you mentioned on the PanOptix. The slightest scratch on my glasses bothers me to the point that I end up removing my glasses, so I don't want to pay $2500 per eye to be frustrated (claustrophobic) with scratches that I can't do anything about.
I have worn glasses since I was about 12 years old. I find there is a certain amount of protection for my eyes in wearing them, so I really don't mind if I have to wear glasses for close up work after the cataract surgery. Would a monofocal lens work better for me? I do a lot of close-up work like sewing and woodworking with saws and power tools, and other crafts. I am myopic with my strongest correction having been -2.25 a few years ago, but it is now at -1.75. I have very slight astigmatism to the point that the dr. said he would not correct it, and a family history of macular degeneration. I believe I have the "macula pucker" or something like that, but not macular degeneration. We've been watching the macula for nearly 10 years. My eyes have always been very sensitive to fluorescent and now LED lights. Oh, and I am 68.
I'm sorry for the long comment and questions. However, I would so appreciate hearing your thoughts on my concerns. Thank you so much!

@A. Aubry it depends what you are looking for. The Eyhance is not a multi focal lense only in that it is still a one piece not a three piece. It is designed to give great distance and intermediate vision but it does not have the same incidence of glare or sun bursts associated with even the premium multi focal lenses. It is pending approval by the FDA but it has shown great results the past three years in India, the past two years in Europe and the past year in Canada. If you’d like to research it or look at some TH-cam videos it is made by Johnson & Johnson Vision.

I drive a tugboat and half the time I'm operating at night with bright lights from tie ups and shore side machinery as well as the lights on the tug. Not to mention navigating near big cities. I've been really looking at the symfony lenses, but the more I research the more I think a monofocal would be best (set to distance) and just wear readers (over 90% of my vision I use for distance).

Great video Dr. Wong! I've researched everything I could get my hands for months and this video explains more in 28 minutes than I've found in six months, and it's in layman's terms. I'm 65 and I had RK surgery back in 1991. I have 8 cuts in each eye. My cornea doctor says I had a "mini" RK, since the cuts were so small. My cornea is clear, but I do have Pigment Dispersion Syndrome and I was diagnosed Thygeson's Superficial Punctate Keratopathy in my early 40's that was pretty severe, but one treatment of steroid drops and I never had another flair up until 4 years ago and it was very mild. I have cataracts in both eyes and am hoping to have replacement surgery next spring. I was also hoping to have the PanOptix lens but was unsure if I could because of the RK surgery. I wear multi-focal contacts, so I'm able to see distance and read. My vision is +2.0 in my left eye and 2.5 in my right. I haven't picked out a surgeon yet, but after doing a lot of research, and since I live in Northwest Arkansas I thinking of using Dr. Steven Vold. Do you think I may be a candidate for the PanOptix lens? I know Dr. Vold seems to be cutting edge in his practice and is involved in a lot of clinical studies.

@@jean-marcdube6954 Thanks for the reply. Perhaps you could have lost more than 8% of light transmission before lens exchange, for example, 15% lost due to cateract cloud. And after lens exchange, you end up with 8% loss due to concentric circles on Syphony lens. You experience more light entering your eyes because you recovered back 7%. Is my analysis correct or wrong?

Sorry Jean Marc Dube, I hope I am not rude or intrusive. I am trying to figure out from your experience to see if there is a way to avoid this 8% loss. And trying to attain the best of both worlds. Meaning getting rid of presboyia + achieving 100% light transmission + solving cateract. Sorry if I sound too demanding or unrealistic.

Dr Wong, thank you for this very informative video. I have just begun to explore this option for several reasons. The information and transparency you provided in this video have great value. At this point I believe I want to come to Austin and have you perform these procedures.
