- In this video I demonstrate how to insert an indwelling Foley catheter on a female patient. I also discuss some tips and tricks and why female patients can be a little bit more difficult then male patients to complete this task on.

Pain is subjective. For some people this procedure would be seen as uncomfortable and others might rate it a 10/10 on the pain scale. If available I highly recommend using a Lidocaine gel to make this procedure easier for your patient. 🙂
Speaking as someone who has been battling a chronic kidney problem since November 2020, has had 4 surgeries, and is having another in a week, I appreciate you mentioning that this procedure can range from discomfort to absolute pain. I’ve lost count of the catheters I’ve had to have in the past 7 months and I can tell you that some times having a catheter placed is a breeze and other times it was so painful I couldn’t even keep it in. You’re an awesome teacher and I bet you are an awesome nurse!

I'm allergic to propylene glycol which is the base of lidocaine. It gives me contact dermatitis. Just as an FYI to anyone putting this on a patient, it would make me raw.

@6:04 Please continue inserting until you reach only about an inch left. A nurse I worked with started to inflate the balloon as soon as she saw urine return. The patient expressed pain. It wasn't high enough.

The opening of the urethra both in a male and female is only a 3mm diameter collapsed slit, and very difficult to find in a female. If you are asked to catheterise an mum on the operating table prior to Caesarean section, be careful not to pass the catheter into the vagina. The urinary bladder must be completely empty to allow operation to commence. Not sure ask for help. Wikipedia UK 'Vagina' fifth photograph down show this opening well, better than in real life.
You must've gone to RN school. LPN's have technique drilled in. I trained Leon's & know all too well that RN's get theory & "on the job training" as far as many techniques.

Amazingly I was a second year surgical resident doing a urology rotation before I ever inserted a foley. It was in the urology clinic and was on a female teen. You can imagine my internal terror at the time. But all went perfectly - thanks to my experiences watching ER nurses do their job years before when I was a volunteer. Those ladies taught me a lot of things. God bless them all.
Imagine all the doctors and surgeons that talk down to nurses (especially when there’s things the nurses know better) and treat them poorly. I know because my wife is a pacu nurse and she tells me about the vast differences between docs. Some are great and even share medical knowledge and others are very demanding wanting things a certain way and short tempered with nurses. Many are condescending jerks. One thing I greatly respect about my wife is having been a SNAP (Student Nurse Assistant Program) employee during nursing school she saw how hard the CNA’s busted their butts and how RN’s looked down on them and treated them poorly and vowed to never be that way. She has so much love for the CNA’s and their hard work for lower pay. The housekeeping personnel too, they just don’t get the appreciation they all should.

I was a nursing student in Sept. 1969. As a first year student I learned how to place a Foley Catheter into a female patient on the Nursing unit, with my Clinical Instructor there to observe and help. We did not have these kind of mannequins in the Nursing Lab. We saw a movie and practiced using the cath kit to get familiar with the equipment. We also used our Nursing Textbooks. I went to a 3 year School of Nursing ! Ah......the memories. I received a excellent education !

I went to a 3 year school too! IT REALLY MEANS real nurses as identifying RN' s . We are last of the caring professionals..I graduated in 1967.

I haven't inserted a foley in about 18 years. My current job has the occasional female come in for a cath for a urine sample to rule out contamination. This is a very informative video, thank you.
Thanks for the informative video! Many of my nurses over the years have gone through this with me too, but I love learning anything and everything. I’ve lost count of the amount of catheters I’ve had over the last 14yrs, they don’t even bother me anymore. I’ve also had drainage tubes in my abdomen after most of my surgeries too. My first drainage tube was taken out before the pain meds had kicked in and I realised for me it wasn’t painful, so I didn’t bother asking for meds after that, just told them to pull the darn things, it’s quicker and easier. When you have health issues, chronic illnesses etc, I think you just get to a point where it becomes second nature and you forget that for most people this isn’t something they are used to dealing with.

Wow this catheter kit is amazing! The ones we have in school, everything is separate. You have to take out the catheter from a plastic and put it in the sterile field. Also the drainage bag are not included in the kit, we normally attach it under the bed before opening the kit. Thank you for this, very helpful!!

I had a hysterectomy last October, also a citocele ( bladder tie up, I spelled it wrong) spent on night in the hospital. The next day when they took out the catheter I had to be able to pee by myself. I could not completely empty my bladder. So I ended up going home with a catheter for 4 days. I was awake when she put it in. There was no discomfort at all. I’ve never had one before that surgery. And I was given instructions on how to take it out. No problems at all. Thank you all that need to know how to do this! It’s so important to us as patients! ❤️💕
You dont show the urethra opening, it looks like the catheter goes right into the vagina.....not very informative
This appeared in my recommendations and I watched because I lost my mom a month ago and one of the hardest things about her hospitalization and home hospice was that she was constantly trying to pull out her catheter. During one of her nurse visits, they adjusted it because they said it was in a very uncomfortable place and she had almost succeeded in pulling it out. 💔 We put the tube under her leg at one point because we thought it would be harder for her to reach, but we didn’t know that could cause a bed sore. She had an iron grip!
After having been a hospital Nurse for many years, I can say that I sure do wish it were that easy to see the meatus- many females are very hairy, or have difficult to see urethral openings…. But, that said, your instructional video was quite well done.

I work in postpartum and sometimes have to insert catheters on women who have very swollen labia. If your patient is very edematous or has a high BMI, I would encourage you to find a colleague to help you with holding back the labia so you can see better. Also, for some women who have had multiple children, the urethra can migrate downward to almost be within the vaginal opening. It's not always where you think it is! It never looks like a hole, it's a tiny collapsed slit that is very easy to overlook. That's why it's a good idea to get someone to help you and to have good lighting. I hope this helps!

Years ago after a hysterectomy the surgeon came in and decided to pull out my catheter WITHOUT DEFLATING the balloon-I momentarily blacked out from the pain. After watching this I’ve come to the conclusion that he was an uncaring, sadistic a-hole. Ugh

a question: can you do this procedure AFTER the general anesthesia takes action? Why to torture and embarrass a patient if we can do it after she falls asleep? is it at all practiced this way? thank you!
Typically the doctor wants everything done before the patient enters the operating room.
Depends on the situation. Not all that need catheters are going in for surgery. So no general anesthesia is being used.

i had a bad experience when my catheter was removed after a operation. The nurse was awful! When she came to my bedside to remove it she didnt say hello just told me it needed removing. As she went about removing it she did not speak just ignored me so i kept quiet. She was trying to pull it out and it just wasnt happening. 5 mins went by and it was really starting to hurt so i said so to which she replied it does not hurt stop being a baby. A cpl more mins went by as she was really tugging on it then she said something under her breath( i didnt deflate the balloon) i knew nothing of catheters so did not know what the balloon was. Anyway she deflated it and it came out no problem. I got no apology and she left quickly with not even a good bye she did not speak at all. The woman in next bed had heard it all she told me about the balloon. I should have complained but i didnt. I got the feeling that the nurse really did not want to be there. No way should that women have been a nurse
Should’ve complained man. You could’ve stopped her from possibly doing that, or worse, or other patients.

Thanks for your Video having helpful tips but one thing to add on is inflation of the balloon immediately after urine comes may end up inflating the catheter in the urethra at the neck of the bladder ending subsequently with complications because the urine come as soon as the tip of the catheter is placed in the lower end of the bladder so it is wise to advance a little more, inflate there and pull back a little to prevent knotting.
So finally I know how to pass folly in female pt 😬

@@rizkamaulita7182 really it does not. The only thing I found out was when I would try and walk a bit the tube would cause some discomfort. Everyone is different and reacts to things in their own way.

It might be a dumb question.... I'm still learning, but why do we have to ask about shellfish allergy? I know latex allergy for gloves something like that. But I don't know what causes shellfish allergies .....? 😔

It is difficult to insert if the patient is uncooperative or obese patient
Sometimes it is, sometimes not.
(speaking only from the patients point of view), when it hurts it's hard to hold still and that seems to make it harder for the nurse too.

Had this once as a kid, hurt like hell and my bladder wouldn’t stop contracting for hours after removal, definitely hurt to pee and it felt like I always had to pee

Anyone, including nurses, that does this procedure. Should have it done to them before they do it to others. They'll understand the pain and pitfalls of it then.

@@eileenmcdonald1599 .No. but I believe those that work with children. Eg care workers and social services, should have had children themselves. As unless you've got children, you can't understand the stresses and strains of being a parent.

I remember refusing to have this done after my first daughter was born. I got up to use the bathroom and passed out. Luckily the nurse woke me up with smelling salts before I hit the ground.
Next kid I got the catheter and years later I still didn't know how they worked so here I am lol. I still don't understand female anatomy enough to figure out where the hole is that it goes in lol.

Three openings; 1) to pee, 2) to poo, 3) for sex, birthing and menstrual fluids. All quite close to each other.

I would continue to hold the catheter until I have inflated it. I have had the Cather slide out and then you might inflate it in the wrong place
Female Foley placement is an art. You encounter every imaginable vagina configuration plus some that just defy logic when you are working. The old adage was always, carry two catheters. Place one.... No urine... Leave it and place the other one in the alternate opening. I've seen over under, side by side, more than two options, and every other kind of anomaly. Fistulas and such make it maddening as well.
Men are typically easier, except when they are really old and have prostates the size of grapefruits. Practice makes perfect, and yes you should have the experience before you go doing it to others. You need finesse and grace as it can be painful to the patient.
