Fascinating choice of subject - the tokkōtai or “special attack units” are the Japanese name for what are popularly known as kamikaze. To the best of my knowledge, the Ki-84 was far too valuable to be routinely expended in these attacks, so I may need to do some digging around to try and find documentation of these examples.
