I love how you show these cool graphs and then give the same answer we all do when someone reports something wonky around 1830 yesterday "it must have just been something upstream" (goes back to working on other things)
Shrugging is a powerful troubleshooting tool.
Your videos are great. I've came across them exactly for the point you made. Checking if my ISP in Bali is trying to kick the can down the road. I am a European living here with a private VPN home server abroad. The performance has dropped dramatically all of a sudden. The ISP came with a new router and blamed it on a broken sea cable in Singapore that I can't find much info on. Still digging into it further and learning a lot. Cheers mate!

What do you suggest for a multiwan on pfsense scenario? A separate smoke ping instance is needed for each wan?
You’d either need to do that or get clever with routing. It might be simpler logically to have a couple of smokepings or investigate the softwares client server model instead.
2 sp instances with a policy based route for each - sp a goes out link 1 and sp b goes out link 2
