In this video, Dr. Deepak Goyal, a well-known Pediatric Gastroenterologist & Hepatologist in Delhi, NCR, has discussed all about - biliary atresia, the main causative factor for direct jaundice.
Neonatal jaundice is of two types: indirect jaundice and direct jaundice. Both types of jaundice are managed differently. Direct #jaundice in children can result in #neonatal cholestasis syndrome (NCS). Biliary atresia is the most common reason for NCS. It is the common indication for chronic #liver damage, #cirrhosis , and liver transplantation. In biliary atresia common bile duct is malformed and this leads to increased bilirubin and ultimately jaundice. It causes malabsorption of fat-soluble components and deficiency of vitamins A, D, E, and K. In the initial days, jaundice is the only sign of biliary atresia. So, parents should be vigilant that if the #jaundice in a child persists beyond two weeks, they should go for a blood test. In biliary atresia, stools become clay-colored, and diaper staining is common. When children come to us with these kinds of #symptoms we have a panel of blood tests to diagnose the tests and sometimes liver biopsy is also done to confirm the disease. If we #diagnose biliary atresia within 60-90 days of birth, a corrective surgery known as a Kasai portoenterostomy can be done.

Sir i m one of the patient of bilirya atresia, i had my opertion done in 1987 , but in 2021 there is obstarction in bile duct so, again repeted opration is done... And stainting is done by dr. Amol bapaye sir. And kasai done by dr naik sir under observtion of dr Datar sir. In kEM , dr, datar sir had operted mi 1987 . So now they recommended for liver trnsplant , so is that ok for mi or not pls suggest .. after 3rd operation stainting i m ok right now , gall baldder also removed ... Pls sugest mi shall we go for transplant or not. My age is 37 now.
If the liver functions are ok - Bil, albumin and INR and clinically there is no jaundice or ascites then no need for Liver transplant.

You need to treat medically first. See if there is any infection. If yes then treat it properly. Look for jaundice and liver functions.
Most of these cases require surgery. It depends on the extent of involvement.

My baby is 65 days old. Recently we came to know that he is suffering from neonatal cholestosis as his conjugated bilirubin is high. But he has a normal stool and urine till now. His ultrasonography also shows normal liver but lft report is not normal.
Is it possible that he have biliary atresia?
There is a possibility. Biliary Atresia should be ruled out in all cases of Neonatal Cholestasis.
You need a Pediatric hepatologist for this as there are many causes of neonatal jaundice and there is a lot of overlap amongst them.

Some babies develop frequent infections after kasai surgery - Cholangitis. Also in some babies bilirubin doesn't come to normal after kasai. There are certain things which can help in diagnosing Biliary atresia- stool color, LFT, Hida, liver biopsy and clinical examination. I think aapke yahan yeh diagnose nahi hoga kyunki aur kaafi diseases hai jo iske jaisi lagti hai. You need an experienced Pediatric Hepatologist, radiologist and pathologist.

I have baby of 41/2 month old. For 4 months he was good without any problem beside skin rash. Five month suddenly he started pooping white. I consulted dr and is uder process for reports and all.. in current report billary level in increased... is it possible in any way without surgery white poop can be normal?
