    So my take away is to strive for homeostatic conditions internally (pain, blood sugar, hormones, hydration, REM, etc) and externally (light, noise, temp, etc) to minimize triggers for sympathetic arousal. Difficult, but worth striving for!

    I don’t suffer from POTS or dysautonomia but I do suffer from tachycardia and PVCs and your videos help me a lot. Thank you.

    Quick question: if you have time, could you do a video on Heart Rate Variability, what it is, and if HRV coherence biofeedback training actually does anything for health? I spent a ton of money on in-person training and devices that supposedly improved my HRV/breathing “coherence,” but this HRV improvement didn’t translate into any improvement in my physical or mental health. I got more benefit from simple mindfulness meditation without specifically trying to control my breathing or HR. So is HRV training just a scam or does it have a real medical basis? I would love the opinion of a cardiologist on this.

    Just know U R helping people even accross the pond. I saw a recommendation to your videos on POTs....Sleeping w\the enemy. I've been watching them & have gained a great deal of understanding. I am fortunate enough to be a patient of the Vanderbilt Autonomic Clinic but have not received nearly as much information to help me as have from you. Thank you so very much..

    Tried everything; nutrition, exercise, supplements, sleep hygiene. None of it works. I am desperate. My sleep doctor doesn’t understand POTS. He gave me a CPAP and I just lie awake all night like usual but now I have a mask on my face. Aaaggghhh...

      Have they tried you on a nightly dose of Clonidine or Guanfacine to improve your sleep? These two help with sleep by telling the body not to release so much norepinephrine into circulation, which in turn calms the sympathetic nervous system ("fight or flight" response). This also makes Clonidine and Guanfacine highly effective at treating POTS - especially the hyperadrenergic type!

    Once again great information! I hope you don't mind - I have a youtube channel which is just a daily vlog about family life and me coping with POTS and other things. I have shared your name and youtube channel info on my videos as you have been a massive help to me - answering questions on live chats and by me watching your videos! I have seen a lot of doctors and none have ever taken the time to explain my symptoms to me like you do - I have lived in unnecessary fear until i found your vids last year!     Thank you for your advice and tips. Only one I’m not sure of is taking a bath. I know it’s relaxing for most people but for me hot water triggers my PoTs. So I’m wondering is it best to avoid to avoid triggering my sympathetic nervous system?

      Me too big time! It causes my BP to drop and I feel awful - I can only last around 5 mins in a bath. I have a shower and was given a shower stool by occupational health and that has been amazing! No more purple legs and skipped beats whilst taking a shower! xx

      Try and take antioxidant supplements an hour before your bath. Part of the temperature change discomfort is due to inflammation( which is one of the main triggers for me in my pots)

      AppleAmor right now n-acetal cytosine is really helping me I take it whenever I'm feeling sick. I would highly suggest bloodwork to see what minerals you're deficient in and then you'll know what supplements suit you best

    Hmm. I’m pregnant (kicking baby and soon to be screaming and breastfeeding newborn), have constant muscle twitching from the Ataxia, and a snoring husband and barking dog to sleep through... chance would be a fine thing! I sleep for only a few hours at a time before I wake fully - Fitbit shows spike in heart rate just before waking so sounds exactly like what you describe but can’t see how I can stop triggers in sleep environment. 😭

    Thanks doc for your helpfull videos... by the way I have a question, can afib patient undergo Total knee replacement surgery ?

      Yes they can. My husband suffers from Paroxysmal AF and had a total knee replacement in August. If this is the type of AF you have, you can expect to have more frequent episodes for a time after the surgery. They put you on blood thinners after a joint replacement anyway, so if you are already taking one, it might just require dosage adjustments more frequently. You need to have your family physician refer you to a good orthopod who will explain it in more detail. I know you wanted an answer from Dr. Gupta, but I thought maybe our experience would help you in your decision.

    Hello Dr. Sanjay Gupta, speeding up ahead sleeping positions...I've heard that lifting the top of your bed in a 30 degree angle will reduce symptoms waking up and during the rest of the day. How is this possible? I mean isn't it also the problem that the blood flow doesn't reach the brain enough when standing what causes a lot of symptoms? So why sleep with your head up??

      From another group. It has something to do with the kidneys and retention of fluid and blood volume... some weird quirk of human anatomy

    Thank you so much for your invaluable suggestions. I have had the worst two weeks and it’s all due to lack of sleep and also stress because when I’m rested I’m able to handle stress better. Lack of sleep, for me makes underlying stress more difficult to handle. During this time. I’ve had severe headaches, depression, mood swings, back and neck pain, and I am also a flight attendant. It’s very, very difficult to perform functions of my job Dysautonomia. I want to cry sometimes because I love what I do but this condition won’t let me be great. I’ll try your suggestions starting today! My husband and I just bought a new mattress. Hoping for the best. Thank you!!!!!

    Can having extraction orthodontics which reduces tongue space affect sleep, due to lack of tongue space in the mouth? Orthodontists don't seem to care to consider the affect extraction orthodontics has on reduction of tongue space. The consequences of having reduced tongue space don't seem to fit into any area of medical specialty. I mean I think my health has been affected by extraction orthodontics I had as a child, and there is nowhere in the medical or dental field I can see who can understand the affect extraction orthodontics has had on my health.

      Orthotropics is possibly what you’re referring to, but it’s not very established in the U.K.

    1. Get more water and salt to drink
    2. Avoid hypoglycemia
    4. Blood test for:
    Vit B12, folate , Iron , thyroid function test, calcium, ( testosterone , if male)
    5. Exercise at evening - 10 minutes of cardio.
    Avoid large evening meal , but don't skip it - just small meal .
    6. Invest in good mattress, pillow, and avoid pain ( take painkillers for pain).
    Improve sleep, with darkness,, no noise,right temperature, no pain.
    7. Period of rest before sleep.no disturbance
    8. Have sleep restrictions, - no gadgets, no stimulants.
    9. Weighted blankets
    10. Sleep positions.
    11. medications

      1. Avoid dehydration. Drink 3 liters of water per day with salt..
      2 . Avoid hypoglycemia. Have regular small meals.
      #3. Have regular rest

    Does napping during the day make POTS worse? I've been napping religiously for over 15 years everyday at around 1-2pm until 4-5pm just for extra energy.

    I have hyperadrenergic POTS and I sleep much better if I jog on the treadmill in the evening. My adrenaline builds up during the day. I take 25mg Coreg and 1mg Xanax to tap down the adrenaline. I usually sleep 4 to 5 hours a night. Fitbit shows I wake up over 100 times a night.

      Wow, do you snore? Have you checked for sleep apnea? I find that taping my mouth at night and sleeping on my side stops my mouth opening causing the jaw to obstruct the airway.

    Have tried all of this... :(. Even sleep meds; Lunesta, Xanax, Trazodone can’t get me to sleep. Nothing works with POTS. If there is something, I don’t know 🤷‍♀️ I wake up exhausted every morning. Even tried the weighted blanket. Didn’t help.

    I have severe insomnia, and I can’t figure out whether the pots (and other issues) caused the insomnia or the insomnia caused the pots ...or if they even have anything to do with each other. Ugh!

      I have POTS and never had trouble sleeping before. Now, every night is just horrible. I can’t get to sleep, when I finally do, the sleep is too light and I wake up too early. My Sympathetic System is always jacked up. I have tried everything and just called my sleep doctor for pills.

    I see a lot of people that don’t have POTS responding. These are good suggestions for healthy people but they just don’t work for POTS. I have tried everything.
