How do you earn the bad ending? Trying to avoid as many spoilers as I can, but I don't want to hit the wrong criteria when going for the best ending possible.

Best ending: Beat the Game with the Feline form
Good ending: Beat the Game with all memories in humanoide form
Bad ending: Forgot your owner
Worst ending: Never recover your memories and died 9 times
