That's how a lot of rappers do it. They'll come up with the flow and the rhythm first before the words by just mumbling, to make sure it vibeswith the beat without even considering words at that stage. But thats just supposed to be the first step in building a track, nowadays a lot of them just stop at the first step and call it a song lol

So this is an actual freestyle. It's hard to think of stuff actually off the top. He didn't slip up, nice work.
And he did this for an hour (twice) with fresh flows, rhymes, and punchlines the whole time. His songs are mostly freestyle work as well with multiple takes cut up into the ideal version of the song. There’s really not many that can truly rival his off-the-top talent
And then there's Harry Mack who makes going off the top look like a cake walk.
