    Very informative video doctor !!
    I have been on topical finasteride for about 3 months now, and havent missed a single day. I notice my hairloss has not gotten better, but has not gotten much worse either. What I also notice, the hairs that fall appear thinner closer the root than they do on the edges. Is that a sign that fin is not working or should I wait for longer?

      Taking Topical Finasteride is just overcoming one's guilt of not doing anything about your hairloss. It does not work!

      @ Doctor, if you mind to explain how that is true, the studies have showed it is just as if not more effective at blocking dht on the scalp. Unfortunately I can not take oral due to complications on the genitals, I tried for one month and experience discomfort which lasted for many months after stopping it.

      It is clear Finasteride is not for you. So you can try topical though I have bad experience with my young patients who were on topical since it is not sufficient to curtail progression to a significant degree.

      @ understood, my goal is to stop it from progressing as much as possible, bringing hair back is not too much of a concern. In my case it is not too visible apart from myself but still enough to cause alot of stress.. I also am on topical minoxidil and do 0.5 microneedle 1-2 times a week, and maintain healthy diet. I am considering trying 1mg fin again since the systemic absorbtion of topical over the course of these 3 months could have been enough so my body gets used to fin and accept it. Would you suggest anything else to help? Much thanks for your reply doctor

      That was a loose term. Yes DHT is a hormone but I was referring to general hormonal causes that cause baldness (not androgenic alopecia) like hypothyroidism, PCOS,
      etc.

    Fantastic video, sir. I watched all of your videos about finasteride. The only person who provides comprehensive information is you. A small doubt, please. I am 40 years Grade NW 3a. I am taking fin 1 mg on alternate days. I also take a 14-day break after every 3 months. It has stabilized thus far. Do you believe that taking 0.5 mg daily will have some advantages over taking 1 mg every other day? I was just curious whether I should try taking 0.5 mg every other day. Thank you, Sir

      Thanks! If you have benefit with 1 mg EOD, stick to it and do not change the frequency/dosage.

      Topical percentage and oral dosage cannot be compared. Topical is of minimal effectivity and if your thinning is rapid or massive, only 0ral Finasteride is going to save you.

    Hello doctor,
    I have a question regarding using dutasteride for alopecia. Since sebum production is essentially upregulated by DHT and testosterone (DHT is more potent), then why did I get extremely oily skin after one month of dutasteride 0.5 daily? This is so confusing because the opposite is supposed to happen! Could you kindly give me an explanation about this issue?

    What would you say about taking dutasteride 0.5mg daily at the morning and saw palmetto 540mg at a late night. I feel like finasteride lost its effectiveness after 3.5 years. Would that strategy be better?

    Dear Dr Bhatti, have you seen a trend of sesonal shedding in your patients? I have been on finesteride 1mg for 3 years now, and last year i started oral minoxidil 5mg and even experimented with dutasteride for 6months. I saw some good thickening of hair. But december 2023 to feb 2024 i noticed lot of shedding and suddenyl my hair was looking very thin, and this was confirmed by even my barber. I have noticed this kind of sudden thinning in february before as well. Is this common?

      Yes seasonal shedding does occur and is not mostly permanent.

    Hello! I did HT a month ago in US. Everything is going fine. I am at grade 7 & age 29. Surgeon not suggesting me to take finastride as i lost all my hairs & finastride has no business left in my scalp. Do you also agree with that?

      Grade 7 means your donor area may not be permanent and for that you need to take the medication or as baldness progresses further, even your implanted hair will be lost.

    I have a question about oral Minoxidil, will consuming caffiene reduce the effects of Minoxidil?
    I heard that aspirin and ibuprofen counter acts oral minoxidil. And that caffiene has similar effects to aspirin and ibuprofen.

    Doctor I'm using finpecia by cipla initially in 3-4 month I have seen some control but in recent days ,my hairs are falling like earlier should I go for propecia which is expensive or stick to the same .
    Note: Now I m using a pill in alternate day , should I alter with daily.

      It's all in your head. Topical is too mild to even have any effect on hair leave alone on your libido.

    Been using topical minoxidil 5% for last 1yr, it showed some good results but after 5months there was a rapid shedding of hair, then Ive been on finateride 1mg everyday for the last 6months, but again after 5months the rapid shedding has started. It's strange how once the shedding is done I don't go though any for the next 5 months. What's going on? It's so confusing. Are the protocols working or not doc?

      How often do you take breaks? DHT blockers do not block all of DHT and that is the answer to your predicament. You need to follow up closely with your treating doctor.

      Topical did not help you the first time. It was something else likely lifestyle change. And now you are having progression of baldness. Topical f is ineffective for unstable androgenic alopecia.

    I'm a diffuse thinner. I've using topical 5% minoxidil+ topical 0.1% finestride for the last 2 years. I had good initial regrowth but in the last 7 months I lost almost 70% of my hair and I'm way below baseline now. Age 24 male. Could it be that topical finestride lost it efficacy?

    If after using both oral Dutasteride, oral Minoxidil, and Keto shampoo for over a year (without sides) but is still losing alot of hair, both visibly and by hair count, how does one know if they should 1.) increase dose of Dut, 2.) add a topical anti-androgen such as Pyralutamide, or 3.) or a form of Estradiol? In watching some of your videos, there are various terms that I don't understand such as 1.) high or low androgen activity, whether increase in scalp testosterone could be an issue for some. I'd like to do something in addition to what I'm doing now but don't want to exacerbate the situation. Any input is appreciated.

    From 2013-2020 I maintained hair well with generic oral finasteride and topical rogaine. After covid I stopped maintaining. I stayed on same protocol for 2 years still losing ground daily. In 2023 I switched to oral generic dutasteride and oral minoxidil (5mg)/day bit after a year, I'm still noticeably thinner and continue to lose ground. Since this protocol isn't working, I'm either going to switch to brand name oral Finasteride (brand Dut too expensive) or topical versions of both. Thoughts? Any input is appreciated.

    Point 7: if the half life of finasteride is 6-8 hours. Then how does concentration peak after 8 hours ? Concentration peaks shortly after the pill is taken. The name half life is there for a reason.

      You may be right. But the exact half life among it's many polymorphic states varies. My videos are simplistic explanations towards ensuring better compliance to medications and ensuring safety as well. It takes 5.5 half lives before the clinically significant presence in plasma diminishes. With most young men rising from bed at 10 AM these days, when do they take the medication?
