Some great tips here. After 25 years as a surgical PA I've transitioned to a walk-in clinic as my "pre-retirement" position. These videos are fast-tracking my rusty cast game. One thing I'd add to the mole skin technique is using an emery board to sand down rough edges. Patients can do this as needed at home, too.
Absolutely loved this video! I was hoping to see a video of how to cast a patient who has pins in their hand as well. If you're able to give me some tips I would really appreciate it.
Thanks for the input! We do not have a video showing how to cast with pins. I would be happy to connect with you and share some ideas and how we do it.
How do you keep your scissors so sharp? After a couple of splint and cast applications, it seems like my scissors can’t even cut the cotton padding during removal
Trev Wilson thanks for the comment...you will notice we use 3 different types of scissors and each has its own purpose to keep them sharp! The key is to not use the same scissors to cut cotton and splint/cast material, otherwise you will not be able to cut cotton after a few splints or casts. The hard part is to make sure your colleagues do the same. Nothing’s worse than when I go to use my cotton scissors and they are covered in cast material!! The cast and splint scissors will require a good cleaning after a few applications, I just scrape them off with a straight end file.
@@mcsmedcastspecialists1285 ...👍🏽 i also keep a steel brush in my drawer(cast room) to clean those serrated tooth scissors, it’s easier. I soaked them in hot/warm water about an hour before I clean them.👌
Doctor, I have a fracture in the neck of the fifth metatarsal of the hand, the ring finger, known as a boxer’s fracture, and I closed it with a splint for 5 weeks without restoring the bone. Does it have any effect or is it due to its function?
Absolutely :) I believe we mentioned it in our commentary, but did not show it. I always put some kind of padding between the digit that will be included in the cast. It will prevent rubbing and the skin from getting macerated. You always want to be mindful of the injury you are treating and use the smallest amount of padding possible. Thanks for your comment and watching our videos!
It depends on which tendon you damaged. This can be used for flexor tendon injuries in the fingers/hand. That would be totally dependent on the physician treating the injury.
@@adelaidebelinsky2124 this cast would not be ideal for an ECU tendon injury. A regular short arm cast or a wrist brace that would allow all of your fingers to be free would be ideal, but that would need to be determined by a physician.
@@mcsmedcastspecialists1285 that is the point he(aka dr. Hamilton) did a cast up past my Elbow and I’m really annoyed with the fact he didn’t even do a proper wrist cast on my wrist
Some areas of concern. By folding the distal and proximal edging the fiberglass is not tethered to remain in place and will tend to migrate and may irritate patient. Also for actual boxers fracture injury the fracture will be reduced and put into Intrinsics plus positioning . Casting from this position is challenging but needed skill to be a proper Orthopedic Technologist.

Thanks for the feedback! An emery board is a great idea. There will always be some rough edges somewhere on a cast.
Really organized and clean work 👌 hope you keep posting videos like that 👑
Alhan Naif thanks!!
Amazing techniques 👌 I learned a lot from your videos
Great! Thanks for the feedback!
beautiful job

Great video
I’ve had this same fracture a couple times and it’s a pain since you can’t use your hand very much
Perfect work. The stockinette you put on the fingers looks much thicker vs the arm. What is the name of that stockinette if you don’t mind me asking?

Would you ever put padding between the 4th and 5th digit to prevent rubbing?

hello sir, how many weeks we use splint for 5th metacarpal closed fracture???
Hello, most of our physicians will splint for 1-2 weeks to allow for swelling to go down and then cast for 3-4 weeks.
@@mcsmedcastspecialists1285 ok, thank you sir

No. 20 some years ago it’s a full cast just in case u fall u don’t fracture the rest of ur bones

This is the most difficult way of applying an ulnar gutter cast
I hate my splint
Alot of unnecessary moves , after all good job
