The little strawberry thingy is to sharpen the straight pins or as they we're called back in the day "common pins"

Hi Barb, my grandmother taught me as a child to “protect the little strawberry”, it removes burrs and keeps your pins & needles sharp too. Love the overlay on the cute tissue box

I would stitch the inside part before cutting out. Gives your foot something more to hold onto.

It’s a needle cleaner/ sharpener….its full of sand I think
