Adhesive Arachnoiditis: Identification, Treatment and Management of Inflammatory Pain

  • Presented by Forest Tennant, MD, MPH, DrPH. Learn about the often-misunderstood diagnosis of adhesive arachnoiditis. We’ll discuss this complex neuroinflammatory disease of the spinal canal covering (arachnoid) and the lumbar spinal cord nerve roots (cauda equina). In other words, two different body parts abnormally “glue” together by adhesions. Pulsed Electromagnetic Energy (PEMF) will be presented as a treatment option, including results of a randomized, controlled trial for patients in pain after lumbar surgery. Dr. Tennant is an internist and addictionologist who has spent his medical career researching and treating intractable pain. He managed the Veract Intractable Pain Clinics in West Covina, California, from 1975 (originally focused on cancer pain and post-polio disease) to 2018. Dr. Tennant served as the editor-in-chief of Practical Pain Management from 2007 through 2017 and continues to hold an Emeritus honor with the journal. Today, he leads the Tennant Foundation and its Arachnoiditis Research and Education Project. Dr. Tennant is a member of the American Academy of Pain Medicine, the Academy of Integrative Pain Management, the American Pain Society, and the American Society of Addiction Medicine. He has authored over 300 scientific articles and books. He formerly served as a Medical Officer in the US Army and US Public Health Service, and was a consultant to the FDA, National Institute on Drug Abuse, and Drug Enforcement Administration, as well as to the LA Dodgers, National Football League, and NASCAR.

    This video was a godsend to me.
    As long time suffer from this condition. It has given me great talking points to bring up at my appointments. I specifically like the point you made about thinking some of your patience were nuts.
    I got mad at my primary care physician for even suggesting it was psychosomatic.
    So thanks again for this valuable information. And for anyone that's
    suffering from any form of this and is just a layman like me with barely a high school education that's done nothing but work hard all their life it's frustrating hang in there and please find the right team of care givers. Don't let them treat you like your crazy.
    When you're hurting that bad sometimes it's hard to be nice.
    Most physicians are trained to treat hostile people as mental and will red flag you in a minute.
    Don't give up.

    This presentation was excellent. I am a nurse and also have developed Adhesive Arachnoiditis. The symptoms are exactly what I am experiencing. I found the steroid packs and Toradol (before I knew I had AA) helped immensely. I am more dependent on opioids ( try to limit myself to 2-3 tabs during most severe acute inflammatory times). I listened to a previous lecture of yours and I am in the process of looking for an expert in Arachnoiditis. That has been a bit hard to locate. I finally found a neuro spine center with providers that are experienced. I worry about using opioids, at the same time I can’t function without them. There is a lot of stigma on medications. My primary doesn’t know anything about this disease. I plan to share your videos. 😊

    Yep. This is what I have it is unlivable. I saw Forest Tennant and he diagnosed me in about 15 minutes by asking a few simple questions and this is exactly what I have. Those symptoms are absolutely spot-on. Urinary problems unable to move your bowels. Sorry. Blurry vision and imbalance. leads to pain like cancer end stage cancer but you dont die. You wish you would. Thank you Doctor tenant for leading the charge. He was so important to this hard-to-treat condition yet the feds shut him down and now we're back. The only problem with this is the inability of doctors to listen and actually learn from him. They just thought he was a kook. No way

    I just got up one morning about 25 years ago and couldn’t get down hallway to restroom. I thought I was just getting some kind of weird flu bug. I only wish that was case.
    About 6 months later I was in a urgent care with weakness and what felt like fish biting or eating at my legs constantly 24 seven as the weakness increased plus if I would let the pain get out of control my symptoms would worsen find a doctor to believe that these days . Fortunately this all started in the early 80s the only fortunate thing about this I guess if you want to call it that.
    Well before
    WAR AGAINST OPIATES
    Now the same doctors that were more than willing to help in part with prescribing pain medicine. Didn’t think the pain was real , bad, or I could exercise and that would make it all better. Here 25 years later just sitting and waiting for enough courage to stand up because then I’ll have to walk into my bedroom to try to get into bed to sleep I haven’t slept a night through since this started I have gone night’s where I can’t sleep at all . I been treated for years with a lot of medication opiates worked best and well for a while then I would have to up the dosage. And that’s when you could get them. Now well I won’t get into that!!!!!!!!!!
    I would tell myself there is no way pain could be so bad it can and will stop you from doing just about anything you wanted , enjoyed had to get done, holidays ,family functions forget about going at least for me.
    I think U get the idea. Usually today my days are spent sitting ,sitting thinking about standing up, but deciding on waiting for a while walking to the kitchen to get a drink of water and then paying the price for making that decision! Wanting to go check mail excited about the order I had made but I I’m afraid I’ll fall get stuck and wouldn’t be able to get up. Sorry I didn’t mean to write a book. But hopefully someone will read this and I won’t feel so alone and lost.

      Mir geht es genauso nach meiner Tarlovzystenentfernung und Thekaloskopie. Es hat die Arach erst richtig angefacht. Ich habe Beckenschmerzen, die Rückseite der Oberschenkel, das Steissbein, Schweissausbrüche, schmerzende Hände, ich kann kaum noch laufen. Sie sind nicht allein.

    I’m in a lot of pain. Okay, I’ll call my doctor. I’m just very pessimistic….. I was at the emergency room recently, and because I did not have loss of bowel or bladder function, they refused to do anything. They didn’t even want to do any further testing. This is public health. Strangely enough, I started doing the rocking and bouncing because it’s sort of helped my back feel a little better. Little did I know that Dr. Tennant is recommending this as part of a self-help treatment.

    I live in Ontario Canada and there's virtually no help for this condition I take gabapentin and Baclofen and Tylenol 3 all in small quantities is there anything else I can take for relief

      I’m so sorry to hear that I have arachnoditis, I have been living in chronic horrible burning stabbing pain 24 hours a day seven days a week! I just don’t know how much longer I can go on like this, I don’t want to. Live anymore

      Ann, so sorry to hear. Last night I was finally able to put a name to my symptoms. I TOTALLY understand how u feel. It is terrifying to know that at some point we will b alone in this decision and act...sadly, others not living this "life", have made it legally impossible to get medical help to end our suffering. So scary to have to eventually do it alone!

      @@williambigg6765 Maybe you can find relief from your excruciating pain do you have the same disease I have of the spinal cord? William please try to hang in there! I’m trying my hardest I only get a very small dose and it’s not enough I’m still in chronic pain horrible pain! If you’re on Facebook look for groups that help Arachnoditis, they can’t really help you but they all have the same horrible disease some have even much worse they’re there to talk with you you could tell your story when you feel comfortable enough and there are protocols to follow Dr. Forest Tennant is in California but he has studied this disease for many years look him up he’s on TH-cam also just go to google put in “traming the most painful pain” I hope this helps you let me know.     I had an epidural whilst being induced in 2013 which was placed wrong the first time, they re-did the epidural and my pain relief was one sided... since then I've had back pain that's slowly gotten worse. I also have balance issues, cant stand sunlight anymore as it hurts and makes me tired, my bones click and ache...GP cant find any reason. at this point I just feel like an old worn out dog that can barely move and needs to be put to sleep 😒

      I’m sorry! Methadone for pain helped me tremendously but my Dr went to a seminar and after coming back took it away. Methadone was originally for heroin-attics but they ( Dr’s found out that it helped cancer patients and chronic pain patients so they started giving it for pain.

I know you all know how much pain people with this condition can handle and it’s a lot but I’ve exhausted everything else, even epidural corticosteroid injections. It’s time for the strong stuff.       Thanks for your video, Dr. I believe I have AA.
      I just wish I could get a post-surgery MRI following my L4-L5 discectomy in March 2020 (which was supposed to be left sided but ended up being bi-lateral). My surgeon refuses to give me a post-MRI even though I am in worse chronic pain post-surgery, falling over again, taking prescribed Fentanyl and walking with crutches for last 18 months, unable to work... I live in the UK and had surgery under the NHS. I'm baffled🤷‍♀️

    Hello i hope you can advise me on something that worrying me.
    Can major inflammation throughout the whole spinal membranes force spinal fluid into the neck?
    If so how can it be drained ?
    Amongst other horrible synptoms the back of my neck is swollen and super tender. I can't have anything touching it ,not even a soft collar as it gets worse and a squishy bulge apears in the side of my neck near my gaw.
    It causes pain in my head, face eyes and ears.
    I cant go to the hospital because my immune system is severly compromise and it's winter . Too scared of making myself even worse .

    Please help 🙏 I'm in so much pain 🥺 it better 10 years and 2 back surgeries later I'm so much worse than I was before. I'm in so much pain. Being crushed from my lower back to my feet. Burned alive and electrocuted 24 7. I can't stand/ walk/ sit/ ride in a car. Drive myself anymore. I can't stand the movement of the vehicle or the bumps in the road 😭

I thought hard work led to being good when you got older.. boy was I wrong about that.. the medical system has destroyed me 🥺 it's all about the money...     Osteomeylitis in my L3 & L4 & discitis caused my arachnoiditis. I had the MSSA type. I have tingling in my lower legs. PT seems to help

    My neurologist says all signs point to this. I’m desperately trying to find answers to the pain I’ve suffered for so many years. It’s hard to pull yourself out of the depths of pain and depression. This is not ordinary back pain. I can’t even cough or sneeze without exploding with pain! I can’t hardly walk anymore and I’m only 42. I use a cane now but the burning and numbness and weakness is so much to deal with!

      @@annm.perrone6089 I’m one step closer to my pain clinic appointment. I’m sure you can believe the hoops I’ve had to go through. It’s time for opiates for me, everything else stopped working years ago. I’m sure my liver is damaged from all over the counter pain medication I’ve taken. I hate taking pills for anything and can tolerate a great deal of pain and still get stuff done but I can barely walk anymore. I’m scared and stressed out of my mind. I have 2 daughters and thank God one of them recently got her driver’s license and now she has to run so many errands to help me.

    Hallo,i am from germany, no Doktor here knows anything about this sickness. I am siffering 10 years, but in the beginning it was not so bad, now the last 3 months getting so woerse. Cannot sit,not laying,not walking without terribble pain. And does anybody also have it with the Darm from that? I have the tarlovzyste s2! Only 1 Doktor knows,but he is so far here in germany away,i am not able anymore to go there!

      Dr. Luchtmann in Zwickau operiert Tarlovzysten. Meine wurden noch durch Prof. Warnke entfernt, aber es ist schlimmer als vorher. Die Operation war nicht erfolgreich. Mich quält die Arachnoiditis genauso. Kann nicht sitzen, habe Darm und Verdauungsprobleme. Ich habe im Juni 2023 einen LP Shunt bekommen, aber nur die Kopfschmerzen sind gelindert.

    Is there anything I can do if I have AA and am already wheelchair bound? I was diagnosed last September and no one seems to know anything, so I've been doing a lot of my own research. A lot recommend standing, but I can't. I believe I developed it from spinal fusion due to my primary disorder which leaves me unable to stand and with weak arms already. I just don't know what to do.

    Will a RFA make your condition worse? I had a RFA on S1 2 3 Dec7th 22 and I'm 6 weeks out.

    I am getting ready for spinal surgery. L5-S1. I have all these symptom, including the running of the water feeling, which I have been describing as buzzing in that area.

      @@mikesanchez9435 I had my surgery 2 years ago. I had a spinal cord stimulator put in which helped a lot especially with my neuropathy. Currently I am render I the S1 area, my chiropractor said it is inflammation. I have developed bulging disk up and down my low spine. I have numbness and tingling in both legs when getting up in the middle of the night. I have started developing scerlosis and have spinal stenosis. Pinched nerve T1. Heading to the surgeons office to see if they can fix me again. He said my surgery will last 15 years. That is better than continuing to take steroid shots. My chiropractor is honest and said he can’t fix bulging disk or pinched nerves. He also doesn’t charge me a “program” just $15 copay for Dr visits. Sitting in bed is bad for me, it cuts my circulation off my legs. I hope I didn’t discourage you, instead do something before you loose function in you legs. That’s my best advice.

      Nerves in the spinal cord that bundled up and stuck together causing the spinal fluid to not send signals, nutrients to the brain which causes life long, severe, debilitating inflammation. This disease is caused by our sedentary lifestyles, degenerative disk disease, pokes in the spine, spine surgery and a certain CT scan that requires injections of non-soluble dyes in the spine. If you get this disease it will be so bad you can’t work. The disease is so bad, it will severely affect your health, income and it will cause early death. This is not a mild disease. It is rearing it’s ugly head again as surgeons preform more unnecessary spine surgeries on our aging population. It’s a unrecognized disease due to the fact many in the medical field are not aware such disease exist.However, the disease has been around since the 1800’s. The purpose of this video is awareness, and getting all in the medical field to recognize such disease exist so they can make the diagnosis and help patients cope with the disease.
