My favorite one so far! I remember some of the patho from nursing school but they never really talked about when it goes from an allergic reaction, to shock and how it affects blood pressure and perfusion. Wow. Thank you Eddie!
Thanks Eddie for all your work, I am a new nurse in the ICU, and your work has been a great help and a great start for me. I feel much more confident. and you are way more better and effectively at explaining and simplifying lessons then my college professors. I really wish you the best in life and in your career.
I went into anaphylactic shock a few weeks ago, immediately after getting an IV radioactive trace injection for PET scan. As I was passing out I told tech “something’s wrong! I don’t feel good. I have heart problems dont give me epinephrine”. I really thought I was going to die and was worried about my dogs being home alone…”Take care of my 3 dogs…get someone to take care of my dogs!” Tech gave me 500 mg of hydrocortisone and I felt better within a few minutes. So scary. It felt like my heart was being squeezed into mush, I could not breath, turned bright red, felt like throwing up/diarrhea, racing heart, passed out briefly and an overwhelming sense of doom. No itchiness , swelling or hives. Several years ago I had a radioactive tracer as part of a cardiac work up. Glad the tech believed me and did not give me epinephrine.
Sir . I am from Bangladesh 🇧🇩 I really like your videos. Sir , Exactly 4 month ago I took a Rabies Ig vaccine then suddenly I feel Dizzy and chest pain .And i was seat down immediately on chair. After 2-3 minutes I started to feel good. I was not going to the hospital this time. I had no skin rash nothing. Just suddenly feel dizzy and had chest pain for 2 minutes. Now after 4 months I have no skin rash. Does it was Anaphylaxis or something? I will be waiting for your Ans. Thank you ❤
Thank you so much for all of your video content. They really help break down such complex topics in ways that are easier to understand. I do have a question for you though in regard to the slowing of the AV conduction with H1… so I understand tachycardia being a common sign for anaphylaxis but then how does the slowing of AV conduction come into play? Wouldn’t that ultimately cause some bradycardia? Or some type of arrhythmias?
Hey Melanie. Great question. So like so much we deal with, its not a simple answer. Essentially though you are correct in your thinking. We do have risks for bradycardia as well as arrhythmias with anaphylaxis. Initially, even before hypotension, we often will see tachycardia though as a response to the catecholamine response.
@@ICUAdvantage lol! Why can’t it ever be a simple answer?? Lol! That’s a reasonable and understandable answer though. So correct me if I’m wrong but basically, AV conduction slowing aka bradycardia/ other types of brady arrhythmias would ultimately be a late sign of anaphylaxis if not corrected in a timely fashion?
Lol is that what we are known as, if one gets a life threating reaction, i like it, although i call myself a complicated patient for hospitals, family doctors, medic drivers. I have had theses reactions which i have explained above. I might start calling myself a shadow person lol but on a serious note, it is very scary when things start to happen and you have only got seconds to let the medical staff know how one is feeling because you know the drugs are lightening fast at putting you to sleep, just as fast as ones own body shutting down on you.
PCWP will be reduced due to decreased volume due to vasodilation. Cardiac output is initially high, then can decrease. SVO2 will be low but can progress to high later on, which is an indicator of increased mortality.
Hi, i have been given epi pens. 1st servere reaction. I went in to anaphylactic shock on a sleep operation, as soon as they put one drug in then another, itchy arm and swelling, i then bolted upright because i was feeling sick, nurse brought a vomit pan, i asked her what was that for, even though i new what it was. She said that i feel sick, i looked at the man who put the drugs in and said do i, feel sick, he said yes, so that is where confusion had set in. He looked at the machine. Then i said My breathing feels strange and i grabbed my chest, then i felt a overwelming dread that something was bad with me, the man looked at the vitals machine again, i looked at him and said, i dont feel well, he said your in anaphylaxis your blood pressure as dropped very low, i need to treat you. I dont remember anything after he said that. On the day i was discharged the Anaesthetist came to me and said You went in to Anaphylatic shock, i repeated it making sure i heard him right . He said yes anaphylatic shock and emphasize on the word SHOCK. He gave me the names of the two drugs that he put in when it all started and said i need to tell people a pharmacy, dentist, ect. Round 2 My second time was an awake procedure, i was only given pethadine straight away i got an itchy arm rash with swelling, i told the doctor and he looked at the nurse who was standing at the side of the vitals machine. Next my breathing seemed like i was breathing through a straw, the nurse looked at the machine, she said shes in Tachycardia, do doctor said he needed to treat me before he could carry on that i was in anaphylaxis. I dont remember anything after he said that, he told me again before i left the recovery ward. Round 3. Was given tablets via hospital Arcoxia, because i now have an auto immune Inflammatory Arthritis which is not old age, like my husband thinks it is. I got a cough and sickly feeling while on them, then my legs pitted edemia both lower legs after taking them for over a week. Told my gp, he told me to stop for 5 days, then start again. My cough, sickly feeling and swelling went almost straight away. I started them again, my cough and sickly feeling started, then and after over a week, pitted edemia appeared once gain on both legs from my knees down, BUT also this time i got an itchy red area on my arm. I phoned my gp which i had to see straight away, he said i was having a reaction to this tablet so he took me off them and told me i cant take ibuprofen, as a pre caution messure. This is when he gave me epi pens to keep with me at all times, because i could have an bad reaction to anything now as im prone to have a servere reaction to drugs. I am having an operation to remove a 6mm kidney stone tomorrow, the hospital doctor,/anesthetist, said i should of been sent for an controlled drug allergy testing, for my own benefit, plus they want to know what other drugs if any i will be allergic to. I have made appointment to see my family doctor about what she said, but in the mean time, she said for my protection they might do a spine injection and put me to sleep that way as this wont effect my air ways if any thing does happen to the rest of my body.
Hi all and pls help me out to understand the situation, I had gone for CT abdominal contrast iv , as dye inserted in my body I start feeling the dye travelling whole body and felt allergy like bee is biting whole body, i stand up and asks rediologist to stop immediately and fainted, after some time when I wake up I peed in my pant and Doctor told me we have given you CPR. Let me know how normal is this ? Can I live life normal and this will not happen again, how is my life expectancy, my all body vitals are ok ,I kept 1 day in hospital to check my vitals. Pls help me out it was shock or cardic arrest. This will definately give me idea what was happend to me actually. Will be appreciated. Thanks God bless everyone
They give me treatment an hour before and an hour after i had ct scan dye put in me just in case i was ellergic to it, as i carry an epi pen because im very bad with some drugs.
My mum died of an anaphylaxis shock and the doctors couldn't find out what she was allergic to be a use an allergic shock causes it, is that normal and is it a pain death, what happens during one.

What about PCWP, Cardiac output & SvO2? Do they change like in Septic shock?
PCWP will be reduced due to decreased volume due to vasodilation. Cardiac output is initially high, then can decrease. SVO2 will be low but can progress to high later on, which is an indicator of increased mortality.
