I worked for PCL on the Vancouver Convention Center project. The absolute worst experience with regards to safety and organization I have ever had. The turnover of employees was staggering. I stuck it out for a year and quit. Beyond horrible.
I was a crane operator for 35 years, Guys will rig the load and new guys seem to have to hold that tag line as you raise the load. Sometimes it's okay but mostly not a good idea. I've hoisted guys off the ground who got caught up in the tag line. Best to stay clear of the load when it's sending.
PCL has some deep pockets too, thank god for his will and ability to hold on…but he will definitely never have to work a day again, or he’ll end up with a nice six figure job at corporate!

The crane operator can not visually see below. They are relying on a radio communications for most movements.
If he had a harness on, I wonder if that would've stopped it?

The guy was working alone when he shouldn’t have been. Normally the swamper is the guy with the radio and designated to communicate with the crane.
