I like this structure but where/ how do you write in any upcoming assessments or tests that the patient needs, their fasting status, elimination or details about discharge / visitors?

depends on the setting, if you are on a ward typically you will be writing notes for 5-6 patients. In an area such as ICU you will typically have 1-2 patients :)
Documentation ...nurses are not data messable anywhere anytime .... reliability is a must have for nurses. Important information is when you are helping a person that's assigned to another nurse and you not assigned there..its job responsibilities ...finishing tasks without delay out of your assignments for that patient, not assigned to you are not your responsibility , and you can document or not document, the assigned should be documenting, you are helping out of courtesy ..... that has to be loudest spelt as injections given should be shouted loudest for the assigned to register check too many times too they might blame if they that are assigned do mistakes to their parents or don't help with empathy ..that's basic. twice recorded or no recording all would be harmful to the patients. Its always the nurse that is more empathetic or working that will be abused by the hypers not finishing their works at time ...that wouldn't finish their own works too in time. If you are smart ..be aware of such too... If you help be really careful. Only those patients that are assigned legally are your responsibility especially in a place that's busy or a day that's busy. Assignments written for you in assignment are only the responsibility . Professional ethics and legal implications are always to be adhered.
