Personal experience, in our case police verification did not happen. Best to give Delhi address at least someone will be there to confirm your identity

Yes, Photo of 51mm by 51mm is required, and thumb is must, i have been returned today because there was no thumb

Yes, only one parent can complete the process, just take all the documents from other parents with signature. Do not forget PPF form
