one trick for palatal anesthesia from old DT is using cold spray on a cotton swab. i practice without a DA, but when i have a DA near me i spray endo cold spray 2-3 seconds on a cotton swab, than the DA apply it on the injection site for 2-3 seconds (but not for a long time because it may cause necrosis- i think- of the palate), swap the swab with my needle and inject slowly a few drops a the site of the cotton wab contact with the palate. sometimes works perfectly but it's not a 100% technique. thank you for this video. hot tooth part was gold!

Great question about the articaine/lidocaine armementarium.
Regarding the hot pulp, I learned some really great tips from Ammar Al Hourani for managing this. Once you have got as far as the pulp chamber, shove in some topical lidocaine on a CWP and leave it for a few minutes. This is 30% so it is stronger and in my experience has worked a lot better than trying to inject directly into the canals which probably washes straight out.
Another great episode! Thanks Jaz and Dr George.

I've got a question regarding intrapulpal injections. If it comes down to the pressure itself and not the anesthetic in those cases, why do we inject anesthetic and not just water/saline/any irrigant fluid and use the pressure from that liquid? 😁

excellent point! I guess in my mind: intrapulpal injections hurt. If the pressure doesn't get it, I am praying the LA will help in some way!
