I recently updated S++ for the (approximate) 2-year anniversary of its release. Many of the older levels have been redesigned and 5 new levels have been added. The player has also been given 3 starting health (up from 1) for each level to make casual playthroughs more enjoyable. Achieving S++ for a level still requires that the player takes no damage (D--). Overall, the quality of this mod has improved greatly, so I would recommend that you give it a try. I think these levels have a lot of potential for speedrunning, but I haven't seen anyone else try thus far.
