I think it might be why at least I, and others in my position that I have talked to, struggle to get proper healthcare. They don't see anything in the scans and the tests. Another ME/CFS symptom can be intolerance of (especially loud) sounds or music or the presence of too many people in a room. Multiple stimuli can be a nightmare causing confusion, irritability and exhaustion. Pacing involves being aware all of these things and essential for any quality of life.

Developmental Sensory Issues. Now just imagine….IF this dynamic occurred during the very young developmental stage of a child’s life, say the first 18 months, all these symptoms would potentially present as signs and symptoms and developmental learning challenges associated with Autism. I wish my GP would watch this! I was advised to do GET and engage with CBT just 2 days ago. When I mentioned my heart rate rises to 120 bpm on standing he dismissed it as anxiety. Hopefully such articulate advocacy will eventually filter down soon. And of course thanks for all your doing, a literal lifeline.
I just want to express my thanks for this video, especially to the two speakers who may well suffer afterwards but I hope not. I could not agree more - covid is a virus and yes it may affect some organs in some of us, maybe because it's more nasty, but it still is a virus and I see myself as a post viral fatigue sufferer and long covider. Given there are so many of us now I actually hope that we will raise awareness and thus help previous ME sufferers by gaining research and exposure. They were ignored for too long and we are in it together.

Well I'm very sorry that the doctor s have fallen ill, I do feel validated that my chronic fatigue, long Covid and all the rest, are finally getting medical attention. I have always said if the world had to live with this for a year or two then all the naysayers within positions of power (medical, legal, aid, families, workplaces) then the narrative and research would change. And along comes Covid 🤷♀️
I’ve had ME/CFS for 23 years and have found very little help within the medical profession beyond the diagnosis. My GP was supportive and sympathetic but had no practical advice. However his initial description of it as starting everything from physical exertion to illness symptoms from a position of physical bankruptcy reassured me he knew exactly what it was like. Much of my help came from a homeopath, an osteopath and my own research looking for boosts for immune systems. I was not given tests for vitamin or mineral deficiency and am grateful to COVID for the lateral thought we now see on management and treatment. Listening to this is so validating snd so familiar, thank you Dr Marsh, we need more spokesmen like you.

I am going through this as well and was diagnosed years ago with CFS. It is terrible because no one understands how debilitating this disorder is. I find myself with no support as I was always the caretaker of everyone else. Unfortunately, I have been abandoned by 'friends' and family (even I do not ask them for anything...it seems they do not like 'unwell' me as I am of no use to them anymore.)
Truth! Same. I was even deemed unfit to remarry in the Catholic Church because I am unable to fulfill my wife and motherly “duties”. Unreal. So much for “In sickness and in health”🙄🙄🙄. The people within our private Catholic school tight community stepped back and away like I had the plague now that I was no longer “able” to attend mass and functions and serve the community. Couldn’t even get a meal delivered or carpool for my kids 🤷♀️. Needless to say, that organization holds no truths. Wishing you well ❤️
I have had long haulers since March. Ive been doing Lots and lots of research on how to get well, or atleast feel somewhat better. Acupuncture has really made a difference for me. I've been there 6 times so far and my body aches have gotten so much better, also my sleep has improved. It also helped with the brain fog! Still working on the energy and my episodes of "crashing". I've also taken 2 doses of ivermectin. I've heard that it has been effective towards getting rid of long haulers. There is a big difference between now and March in how I feel! I just thought I would share what I've been doing incase someone else is looking for suggestions.

@@alexlee8639 I took it by mouth with some peanut butter. The taste is pretty bad. I just bought it fleet farm, a store that sells animal products etc. I bought it for 3.78. Yes, it's that cheap. Thank you to both of these doctors for giving up their time and energy in order to provide further understanding of the physiological basis of ME/CFS, and long covid, and the dangers of graded exercise therapy.
I don't seem to have any PEM. My fatigue can disappear for weeks and then reappear. But honestly, it's nowhere near as bad as it was last summer. Last summers fatigue made it really hard to even try and get dressed.

The emotional connection is vastly over looked and disregarded. I had an emotional event in January that set off a cascade of symptoms for over two months I was debellated every attempt to gain upright posture resulted in relapse. It was hard to get up enough collected fortitude to call in sick for work. I know that sounds funny and it truly is if it weren't absolutely for real I would not believe it myself so I appreciate all those who have humored me with my condition though they don't believe the litany of my complaints. My worry is the increase pull of gravity on me during fatigue episodes... this is accompanied by time delusions where it seems clocks stop and lose synchronization, run backwards as pain hits it's horrid crescendo, very inconvenient for setting a schedule when my own internal clock is out of step.
can only second this. got covid late december, just after i left my bed after two weeks with mild symptoms, i lost a brilliant friend of mine in a tragic accident. the first two months after that even i was a mixture of numb and hyper. it was only in march when the long covid symptoms aka fatigue adn dereaslisation kicked in, as well as breathing problems. i blamed the event entirely and forgot about covid mess. now at 6 months i have the worst releapse ever, bed bound and problems with blood circulation/dizziness. i will follow all the great tips, go to acupuncture, sacro caral therapy and see what happens. we will be fine, but we got to have patience

Vagal nerve stimulation ( lateral eye movement exercises) help...the gut/ brain connection I suspected goes through our eyeball sockets (conveniently explaining how our eyes can be bigger than our stomach) balances our immune responses and all internal metabolisms. Deep Fasting, gentile yoga, breathing, singing/ chanting/ gargling, light meditation... make a difference

Thank you for this...I am a 51 years old woman from Finland and I have suffered from a very severe cfs/me over 21 years and just been laying on my sofa alone trying to stay alive. My neck was broken and I also had pneumonia after a car accident in 1999...my fatigue and other symptoms started soon after that and never been okey since then. @@nobleargent6823 thanks for the links. yes I am aware of that but it is very frustrating because I don't have money for private doctor and I haven't met any doctor in the public health care...not even private doctors know much about cfs over here and if they do they only speak about GET therapy . The only positive thing has been that a private doctor read about low dose naltrexone because I actually forced hime to and he finally prescribed it last year and that is very rare here in Finland. It has helped for my severe cognitive fatigue...without it I wouldn't be even writing this. If I one day meet a doctor that is willing to listen I will talk about that for sure :)

He had 12 months after the initial viral illness during which he was able to function normally at a high level. In my case I only had about 7 weeks where I was able to function, but all the while feeling that something was very wrong. My timeline as follows: End of FEB 2020: Flu-like illness, after 4 days recovered 2 Days later: Sudden insomnia, feeling unwell again Next 4-5 weeks: Lots of fatigue, malaise, other dysautonomia symptoms Following 6-7 weeks: Partly back to office, gradual improvements, but still poor sleep and other symptoms Then I crashed with severe insomnia, worse dysautonomia, PEM, the whole lot...... Now 2.5 years later I'm a little better, but far from recovered.
Hello all, am an M. E. sufferer for the last 21 years and as of April 2020 covid long hauler as well, since it was a nice afternoon I thought i'l have a little walk(nicely paced) down to the shops, I am back now, feeling nauseous, fatigued and shivering just as I was 14 months ago, given my own experiences with both for the last 2 decades, I am now firmly of the opinion that both of these diseases stem from viral persistince. The only way to be “treated” is to completely change your lifestyle and what you eat. There will never be some magic prescription. The long covid problem is systemic and complex rooted in inflammation and other chronic infections.
I have been hoping for this too. What i find concerning is the not wanting the two groups to be considered as having the same problems. Separating the two groups is not good news. Long haulers have a real problem, it's because of the virus and the other group, still don't actually have a problem, it is all in their head. Being a sufferer of CFIDS I have been thinking about a pathogen or , vaccines triggering similar symptoms as long Covid. Inflammation could be the the common denominator causing other illnesses.
I had Covid (mild) March 2021, minor long Covid symptoms. After my employer pretty much forced me to get the jab in Aug, my long Covid symptoms exploded, extreme fatigue and brain fog. My quality of life has been extremely reduced, because of the experimental drug they are calling a vaccine

Dr marsh, I can’t thank you enough for sharing your story and experience managing post viral CFS. I am 14 mos post Covid and struggling still with too many symptoms to list but the worst is the fatigue and PEM. I am working on pacing and learned so much today from your story. I’m very emotional even writing this because the validation that the PEM is real helps me so much. I’m surrounded by well-meaning friends and family that think I’m still sick because I’m not getting out enough and exercising. I've gotten better at managing it tho - planning for the PEM and saying no to activities that I know will be put me out of commission in the days following. Its a complete life changer - I use to travel 3 out of 4 weeks for work and now I struggle even working remote. I'm trying to stay positive but there are some days it feels overwhelming. I started crafting as therapy :) when the covid insomnia hits I get out my craft supplies - reserve computer for work effort - day by day. @@MK-gy1ug Thank you. I appreciate the encouraging words. If you are 13 weeks post infection, the most important thing is rest. Honestly, I got COVID in Oct 2020 when there was no vaccine and no treatments. There was little to no understanding of recovery after severe infection. I was critically il for 30 days and ran a 103 fever for 12 of those days. After 6 weeks I returned to work (remote) but I was still so sick. By Christmas of that year, I was losing my hair and developed a whole new host of symptoms. I was so sick but still be scheduled to present at conferences virtually. The people I worked with were supportive but thought because it was remote work that it wasn't taxing. The brain fog really kicked in around month 3 and the fatigue was just debilitating. I kept pushing it through it but one day I fell asleep during a web meeting. Completely out. I tried to slow down but there were so many people sick with acute COVID and I felt guilty saying no to meetings. I'm a nurse - we just keep going till we fall over. My days morphed into 12 hours then 16. By the following summer, I was so sick I just couldn't do it anymore. My employer told me to take the time I needed only to have everything waiting for me when I got back. I couldn't sleep. I couldn't eat. The joint pain was debilitating. I finally made the heart wrenching decision to resign in Sept 2021. Only a year before I was named one of the top 25 female executives in healthcare IT and now I was resigning. I took 3 months off and it really helped. My hair grew back :) I gained some of the weight I had lost. I was able to sleep a little better. Since then, I've learned how to better manage the energy I have and plan for the post exertion malaise. I started a new job in january that was less demanding. That said I'm struggling even with 8 hour days. I take things day by day and hoping they will find something to help us, but I do believe that if I would have let myself recover fully in the months following my acute infection that I would not be this bad. I had no preexisting conditions. Healthy 55yo until COVID. So please MK - give yourself time to recover. Rest, hydrate, eat and clear your mind to focus on healing. It comes and goes but it’s really horrible when I do have it. I feel like my arms are going to fall off from pain and weakness. I see some sites telling me that I should build my strength slowly by walking a little bit each day, just so I don’t sit in bed all day And get atrophy. Then there’s other sites that say I should be resting all the time so I can avoid PEM. Not sure what the right thing to do is. I have kids which means resting can be close to impossible sometimes. But I do the best I could do!

The reality is that only around 5 per cent of people with ME/CFS recover. Dr Marsh isn't being negative. He's simply describing the reality of living with this condition & how best to manage it until effective treatments come along.
@@FionaEm I’ve read that statistic too but I don’t believe it. Where does it even come from? Who is keeping track of those who have recovered? I believe it’s like the 4 minute mile - once you believe it’s possible, that mindset will help you recover. Mindset alone isn’t enough, but not believing that recovery is likely will certainly hinder you. I know what a horrible illness this is. It really wears you down and tries to steal your hope. I’d like to send positive vibes to everyone with long covid / ME/CFS. I’m almost recovered now, 16 months after my initial illness and 5 months after getting an NLP coach to help with my mindset. Great interview that reflects very well the relationship between the illness and the medical community. And yes, gradually increase you to your triggers has now been proved to not work. Go for about 50% of your capacity so you have some margin. Pacing as mentioned is the way to go
Get a Garmin watch. It helps you read the "body Battery" and indicate stress Level. For me a game changer as it helps me to not over do things. I have alarms on heart rate and when I achieved 3000steps I make sure I stop to not get hit with post exercsion malaise!
I do this too. The visible app is releasing a polar arm band that measures HR and HR variability all day. It will apparently tell you when to rest etc. designed for long covid ME/CFs. Have you heard of it?

Covid is tragic with the deaths it caused. For ME/ CFS and Dysautonomia sufferers it has made that tons of money is being distributed to medical studies. It's a game changer for us who have been suffering with this for a long time.

I am 14 months post covid (a very mild case) and have struggled with the exact same cycle of 'activity' (physical/emotional/mental) induced relapses as you describe (+ a number of other symptoms). I was extremely active before all of this and my fear now is that I may never return to my 'old self'. I live in hope that there will be a treatment found soon to help us with these life altering symptoms. At least until them I can now pursue support for ME/CFS + PEM.
I take a collapsable chair for my short slow walks. I pull it out often during the walk, and sit down for a longer time than I walk. That rest is so important. I also drink more around the time I exert, so I piss out anything the cells are excreting after metabolising. As I believe the immune system is attacking these molecule. So pissing it out early I think helps. Although not enough
Andrew, sorry to hear about your CFS. I’ve actually had Fibromyalgia (FMS) and CF for the same duration. I’m only in my 8th month of the joys Long Covid. No two days are the same regarding the reactions/symptoms. Variety is said to be the “Spice of Life”, but I’m not convinced in this case! 🤗😉😉😂 Although no help to you, I can completely sympathise with you regarding the fatigue and brain fog. Good luck. My symptoms are getting worse as time goes by, I also developed lower and upper back pain suddenly with this with most of the symptoms this Dr has. The fatigue is crippling .
Thank you for these long hauler videos!!! The ones about POTS and MCAS really helped me get on a path to feeling better. I’m not 100%, but able to function and feeling so much better. Learning about Mast Cell Activation has taught me how to tackle this. Hopefully I won’t have to be on antihistamines forever, but they have been a lifesaver. Keep doing these videos. Very touching story.. As he said, "fatigue" is really a misleading word to prescribe this illness. However, I wish he had talked about some of the available medications to manage PEM (niacin - monolauric ..etc), and his opinion about their efficacy. Thanks for sharing this interview!

Excellent video, thank you both so much. It is really important to understand that exertion is *not* just physical. My sister has been bedridden with Long Covid for months and we've done a lot of learning - absolutely key is that emotional stress, cognitive load and social interaction can have an even greater impact than physical movement, as these actually use a lot of energy. Even positive social interaction can have a detrimental effect on your energy, which is really hard to come to terms with...you have to trade-off, because no social interaction will of course make you more lonely which is bloody horrible! Another crucial thing we learnt was understanding that the 'baseline' activity that doesn't cause PEM doesn't mean the level of activity that makes you symptom-free. We kept thinking that what my sister was doing couldn't be in her baseline because she was still so ill but once we realised that we should be looking out for that flare up of flu-like symptoms it became clearer. So it's about finding what you can consistently do each day without getting significantly worse 1-3 days later, but also realising that there are things outside of your control that will use up some of your energy (e.g. a heat wave, menstruation, a call with your Dr you have to do) and so when these things happen you might need to ensure you're resting more and doing less in other areas than you usually do.
Your symptoms reflect mine so closely. I have tried and thought I had found a way to manage it but not. I have 2 - 2 1/2 hours window to do my work. If I go over that I am unable to function for the next day and sometimes longer. Nobody understands this. They think I have given up or have become lazy. The harder I fight it the harder it fights back. It is now mentally destroying me. I have been a self employed engineer for many years and have carried out many demanding projects and tasks alone and love a challenge. This is a challenge that is beating me. It is destroying my life. Given your condition iam surprised that you can talk so articulately for so long. I find my speech starts to slur and my confusion stops me completing sentences after around 45 mins. Having been unaffected by the covid infection I struggle to accept how it has now ruined my life.
As a covid long hauler since last November, I was actually quite happy when my doctor immediately diagnosed me with chronic fatigue syndrome because of my post-exertional malaise. It gave me something that I could research while they were looking for covid specific treatments. I'm lucky enough that I think I have figured out my post exertional malaise triggers. For me it is definitely heart rate and anything that requires an increase beyond standard oxygen levels into the cells. For example, I can go for a 3-hour walk if I walk slowly, but a 20 minute walk at a fast pace will cause weeks or months of PEM.
@@murielh7678 I would describe it as waves stacked on top of each other. A fast walk might cause a medium wave that lasts 3 weeks, carrying a heavy object might cause a large wave that lasts 6 weeks, and trying to hold my breath would cause a small wave that lasts 2 weeks. At any given point, my current feeling was the sum total of all current in progress waves. I stopped experiencing PEM waves after about 6 months, but then went on to develop histamine intolerance which causes its own set of waves, but those are much smaller and more manageable as they just lead to mental confusion and memory issues, not complete multi-day collapses into bed. The major thing I attribute to the end of the PEM waves was figuring out that I had intestinal candida overgrowth, and going 100% gluten and sugar free, and getting stool microbiome testing through a naturopath until the candida came back negative (no candida growth observed).
This is an incredibly insightful video especially with that interview of the paediatrician. I have learnt so much more about long covid is, not just a condition with a name, but, that it is DEVASTATING for the individual’ s life, and living; as well as for his or her family and friends. This brought tears and terrible emotions of sympathy I have not felt since the pandemic struck. I am grateful for this insightful education. Thank you.
Thanks for this conversation about PEM and GET. I'd love to know if any longcovid experts might have insights into taking targeted longevity supplements to address specific longcovid symptoms (such as neuropathies). I'm in the pacing and rest group, for sure, and am feeling better when shopping the imaginary longevity supplement shelves to address, for example, GI issues (butyrate & IGG), or neuropathies (alpha lipoic acid and hyaluronic acid). In addition to targeting my specific issues with supplements, I'm also feeling better now that I've backed way off on all exercise beyond daily house and garden work. I've heard some athletes with longcovid were instructed by their coaches to take 90 days off all exercise, so I'm giving that a go, and so far feeling the best I've felt in the last 16 months of my longcovid journey.

I've been taking Glutathione and Quercetin and Zinc as well as many others... the recent add-ons are those that are giving me noticeable huge improvement to my GI and neuropathy issues that seemed to hang around and worsen on relapse. I haven't worked with the other aspects you mention, but I'm so relieved to see such positive improvements in my GI/gut issues by simply adding to the usual MCAS and NAD+ stack with a few personalized supplements. I'm still not recovered from longcovid (still can't exercise as before), so for that I'm backing further off all activities. I am learning that left to my own devices, I tend to be an overachiever, and that is not helpful when it comes to pacing and rest. I love to keep learning, thinking, doing, and have to teach myself "this too is a sick day--take it easy."

I've messed around with numerous supplements. Mostly unsuccessfully. The notable exception was gut bacteria. It didn't really make me feel any different but my poo became way better and normal again. So it kind of helped alleviate just one symptom. However none of it really made me feel much better in terms of energy, concentration etc. Just better poo.

I have had cfs for 20+ years. NOt even diagnosed officially, let alone helped or supported by my country's equiv of NHS.. I was looked at by some Dr's as a hypochondriac, 'hysterical', 'just' depressed late 30's/early 40's woman. I used to be a Reg Nurse. My hubs kept telling me until a Dr gets CFS ( or now Long Covid) they will not care abt it .My hubs had open heart surgery last year and within a few weeks of the op, had more energy than I did. he was back at work after a few months. I have not been able to work for over 10 years.
Excellent video again! I relate to Dr Marsh as I too am housebound 99%. The point I would make is that GPs are still in the dark with ME CFS and I think they choose that. Any help I have received has been at personal expense, GPs simply are not interested! I think treatments for Long covid may come along, but those with ME CFS will not receive or benefit. I've battled just to get antihistamines prescribed and paid a private doctor. The NHS have only one concern, minimising costs.
I will get 2/3s into a 'normal day' and it will start. I will feel it behind my eyes first. If I don't lie down and sleep, I feel like I'm going to die. Sometimes I will wake up wondering what day it is. It is often like 12-14 hours later! For much of the pandemic, all I did was work (while feeling badly) and sleep. It feels like I have lost 3 years of my life. Ironically I had to work harder often, because I had to teach both online and in classes. But I had nothing for teaching online. I would spend many hours creating materials for online. All I did was work and sleep. I don't remember enjoying much of life at all.
Really good series Gez, guess I’m not the only one who took up Duolingo and learning Chess online then, and even got a slack line with my kids to begin some form of exercise. Still got the chronic fatigue, brain fog and exercise intolerance but the recurring bouts of flu have subsided somewhat when I have a relapse. Been referred to the CFS/ME clinic, overlap of symptoms are very close, just wonder that the pathogenesis of long Covid is maybe different for us.

IAM a qigong & taiji instructor and a retired nurse and midwife. I am wondering if, in the area of rehabilitative exercise, researchers have looked into the science of qigong & taiji, that may benefit people living with long covid ?
The survey mentioned was a questionnaire done by the ME Association, given to people with severe ME, about their experiences of treatment. You point out that the draft guidelines were supposed to be finalised in April, but that this ‘has been delayed’. I would think it likely that the powerful psychiatric lobby in the UK are making a last ditch effort to have CBT and GET reinstated. There is the threat of reputational damage and possible even of legal action taken against them, especially if it were shown that they knew of the harms and still prescribed GET. They will be doing everything they can to block these proposed changes to the Guideline.
I'd be curious to know if the community has any experience in the following: For me (along with the usual symptoms described here) it's as though my circadian rhythm is now broken. If I change my productive period to 1am to 7am and sleep till 3pm, I have so many better, productive, almost normal "days", compared to trying to be awake and functioning at the same time as the rest of my time zone and suffering the debilitating effects of this.
This has definitely been my experience too. To get myself back into a regular sleeping schedule I've been taking melatonin, magnesium, gaba, and Valerian root. I need to take about five valerian root pills with that combination for it to work.

I like others have tried to describe my symptoms to a my cardiologist and to the nurse practitioner of a covid cardiology specialist no one seems to be listening I had post covid pericarditis was prescribed beta blockers but the made my tachycardia go from tachycardia to bradycardia to say drop my pulse to 43 which made me lightheaded and feel like I was going to pass out. Now they want to try calcium Chanel blockers i dont know whether to try this or not. They want me to do the exercise program I walk at least 5 miles a day as a nurse the fatigue is very disabiliting the nurse practicer said this exercising of walking at work does not count
I read that this virus appears to thrive (and replicate) with exercise-induced oxidative stress, is this true? If so, should you not exercise. I get out of breath very easily during exercise but feel fine when I finish.
@@RUNDMC1 But if the virus lays dormant I could avoid exercise for 4 months then go back and be exactly where I was 4 months ago. Do you not think our bodies will deal with the virus regardless of exercise or not.

I am taking Nadh, coenzyme q10, b50 complex, melatonin, magnesium, zinc, selenium, quercetin, cod liver oil, omegas, and some other stuff. It's all helped with my baseline energy and mental state, but nothing is preventing post exertional malaise at this point.
I have been struggling with all this 5ys but being female,nurse, over 50 was laughed at then muscle biopsy said IBM, which doesn't have these sx. I pressed for mestinon despite being double sero neg. In one day eye pain, diff focusing, pain from muscles collapsing, difficulty breathing all eased. Neuro still not convince can have 2 rare diseases but keeps writing mestion which has more than doubled single breath count. I did have covid last yr in March when all was chaos and luckily treated myself at home with ordered bipap and sl inc FiO2 to stay above 91. 5 mos later could get dressed and go out if someone else drove and used neck pillow, cane. I am just wondering if Mestinon might... did see it is used sometimes in dysautonomia which surprised me. Neuro isn't upset my HR goes from 49 to 104 for no apparent reason, or that mental stress can wipe me out as well as walking and a cell phone is too heavy at times.
Thanks to both of you for this moving helpful film. I have a question - with POTs the recommendations seem to suggest that recumbent movement is helpful to start and then a slow gentle progression to more upright movement and exercise will ultimately help reduce symptoms but........when POTs is a secondary dysfunction like in long covid or other viral illnesses it seems that exercise can be damaging so.......when like me and many others I have POTs and clearly post viral exhaustion from long covid it seems to me that the deconditioning due to having to rest loads to help with the long covid is not helping the POTs at all - would welcome any thoughts but is this maybe why the POTs is not improving in those of us stuck in this position I wonder?

Mary Ruddick Nutritionist Firstly - the importance of convalescence, and that cognitive effort can be just as bad as physical exertion. Then the role of a low histamine diet and antihistamines for those in the MCAS phenotype. Pacing is absolutely key and not rushing back to work. Some with NAD+ deficiency respond very well with niacin supplementation.
This information along with Dr. Bruce Patterson’s discussion of how viral fragments ingested by monocytes in the blood stimulate an immune response (inflammation) wherever they go, and that this harm is increased when exercise stimulates these monocytes to go further into the system is an absolutely clear justification for dismissing Graded Exercise from the revised Guideline. The doctors, especially paediatricians who are currently having publication of the revised Guideline are insisting on retaining the power to impose severe medical harm on patients, including children.
It seems a problem for some that this diagnosis is 100% excluded if it starts in childhood (or even adulthood) and without diagnosis because it is no longer possible to "never occurred" previously. What designation can describe ongoing episodes?
If you haven't read "Plague of Corruption" by Dr. Judy Mikovits and & Kent Hechenlively, JD, you should. While it gives no solutions, it does expose the lack of progress due to systemic corruption in this field. I too have been suffering with ME/CFS now in tandem with post covid fatigue. I'm finding the fatigue incredibly debilitating. I'm six months in with long covid . Are you all seeing improvements now in your fatigue ? It's the pacing that's difficult . I went for a walk Monday and I felt good and it didn't over exert me but then by Wednesday , all my symptoms flared up with chills , aches , fatigue and IBS . Will the systemic inflammation eventually go away ? I thought I was in tune with my body but clearly I pushed it too far . I've relapsed for days now.
Hi Ella, the trick is not to push it too hard on the good days. You’ll feel fine at the time but it will bite you later. 6 months in was also the worst period for me so part of it is also the course of the illness. Be patient! :)
I’ve had the same symptoms since my second Pfizer . Glad to learn more about it. Poor Dr Marsh … you could see him getting fatigued and brain fog about half way through.
I have Long Covid and really not well, had a really bad week and my daughter ended up taking me to the hospital....the doctor told me I didn’t look ill and he had had covid and he had recovered, and if I wanted to see poorly people he could take me to a couple of wards....
Can post viral fatigue linger post six months but not be defined as CFS/ ME? . I am trying to hold onto the thought that I will recover from these symptoms. I've read that only around 13% of people recover from CFS hence why I'm really hoping my body will recover from this illness . I'm almost six months in though and the fatigue is still persisting as are the mental /physical exertion issues
Yes, chronic fatigue syndrome is a terrible name. The illness is a post viral disease which includes dysautonomia, POTS, NMH, other forms. Brain fog is another symptom. Post exertional malaise which is mental, emotional and physical exertion. Practitioners still encourage exercise which is known already to cause further damage. There is Hope . Joining together to learn is important , there is much proven evidence available.
My experience of ME is to sleep as and when you need to. If you cannot sleep, meditate and do deep body relaxation instead. It may help during the day to try and keep more to deep body and mind relaxation if you can as that may help you sleep at night. If you have to be up at a certain time, allow an hour of deep relaxation and diaphragmatic breathing beforehand and drink two large glasses of cold water to help with orthostatic intolerance. One of these preferably being a rehydration drink. During the day it may also help to lie down frequently with pillow under knees to secure blood flow and oxygen supply to brain. I combine this with relaxation exercise and slow diaphragmatic breathing for emergency recovery

I think it might be why at least I, and others in my position that I have talked to, struggle to get proper healthcare. They don't see anything in the scans and the tests. They just see the window where I am calm, well spoken and is functioning enough to sit in a chair and explain how hard something they can't see is. They don't see how it took me days to prepare and how I the day after will lie in bed and be physically unable to lift my arms.

Another ME/CFS symptom can be intolerance of (especially loud) sounds or music or the presence of too many people in a room. Multiple stimuli can be a nightmare causing confusion, irritability and exhaustion. Pacing involves being aware all of these things and essential for any quality of life.

Developmental Sensory Issues. Now just imagine….IF this dynamic occurred during the very young developmental stage of a child’s life, say the first 18 months, all these symptoms would potentially present as signs and symptoms and developmental learning challenges associated with Autism. I just want to express my thanks for this video, especially to the two speakers who may well suffer afterwards but I hope not. I could not agree more - covid is a virus and yes it may affect some organs in some of us, maybe because it's more nasty, but it still is a virus and I see myself as a post viral fatigue sufferer and long covider. Well I'm very sorry that the doctor s have fallen ill, I do feel validated that my chronic fatigue, long Covid and all the rest, are finally getting medical attention. How are u feeling today with long Covid fatigue?
I’ve had ME/CFS for 23 years and have found very little help within the medical profession beyond the diagnosis. My GP was supportive and sympathetic but had no practical advice. However his initial description of it as starting everything from physical exertion to illness symptoms from a position of physical bankruptcy reassured me he knew exactly what it was like.
Much of my help came from a homeopath, an osteopath and my own research looking for boosts for immune systems. I was not given tests for vitamin or mineral deficiency and am grateful to COVID for the lateral thought we now see on management and treatment.
Listening to this is so validating snd so familiar, thank you Dr Marsh, we need more spokesmen like you.

I am going through this as well and was diagnosed years ago with CFS. It is terrible because no one understands how debilitating this disorder is. I find myself with no support as I was always the caretaker of everyone else. Unfortunately, I have been abandoned by 'friends' and family (even I do not ask them for anything...it seems they do not like 'unwell' me as I am of no use to them anymore.)
Truth! Same. I was even deemed unfit to remarry in the Catholic Church because I am unable to fulfill my wife and motherly “duties”. Unreal. So much for “In sickness and in health”🙄🙄🙄. The people within our private Catholic school tight community stepped back and away like I had the plague now that I was no longer “able” to attend mass and functions and serve the community. Couldn’t even get a meal delivered or carpool for my kids 🤷♀️. Needless to say, that organization holds no truths. Wishing you well ❤️

I have had long haulers since March. Ive been doing Lots and lots of research on how to get well, or atleast feel somewhat better. Acupuncture has really made a difference for me. I've been there 6 times so far and my body aches have gotten so much better, also my sleep has improved. It also helped with the brain fog! Still working on the energy and my episodes of "crashing". I've also taken 2 doses of ivermectin. I've heard that it has been effective towards getting rid of long haulers. There is a big difference between now and March in how I feel! I just thought I would share what I've been doing incase someone else is looking for suggestions.

@@alexlee8639 I took it by mouth with some peanut butter. The taste is pretty bad. I just bought it fleet farm, a store that sells animal products etc. I bought it for 3.78. Yes, it's that cheap. I took a dose for a person of 200 lbs. Watch the self sufficient momma. She has a video on how to take, and how much etc.

I don't seem to have any PEM. My fatigue can disappear for weeks and then reappear. But honestly, it's nowhere near as bad as it was last summer. Last summers fatigue made it really hard to even try and get dressed.

The emotional connection is vastly over looked and disregarded. I had an emotional event in January that set off a cascade of symptoms for over two months I was debellated every attempt to gain upright posture resulted in relapse. It was hard to get up enough collected fortitude to call in sick for work. I know that sounds funny and it truly is if it weren't absolutely for real I would not believe it myself so I appreciate all those who have humored me with my condition though they don't believe the litany of my complaints. My worry is the increase pull of gravity on me during fatigue episodes... this is accompanied by time delusions where it seems clocks stop and lose synchronization, run backwards as pain hits it's horrid crescendo, very inconvenient for setting a schedule when my own internal clock is out of step.

can only second this. got covid late december, just after i left my bed after two weeks with mild symptoms, i lost a brilliant friend of mine in a tragic accident. the first two months after that even i was a mixture of numb and hyper. it was only in march when the long covid symptoms aka fatigue adn dereaslisation kicked in, as well as breathing problems. i blamed the event entirely and forgot about covid mess. now at 6 months i have the worst releapse ever, bed bound and problems with blood circulation/dizziness. i will follow all the great tips, go to acupuncture, sacro caral therapy and see what happens. we will be fine, but we got to have patience

@@nobleargent6823 thanks for the links. yes I am aware of that but it is very frustrating because I don't have money for private doctor and I haven't met any doctor in the public health care...not even private doctors know much about cfs over here and if they do they only speak about GET therapy . The only positive thing has been that a private doctor read about low dose naltrexone because I actually forced hime to and he finally prescribed it last year and that is very rare here in Finland. It has helped for my severe cognitive fatigue...without it I wouldn't be even writing this. If I one day meet a doctor that is willing to listen I will talk about that for sure :)

It's very shocking but instructional to see Dr Marsh sometimes struggling with the words, the message he wants to share.
I don't think that he was so "slow" before long COVID.
He had 12 months after the initial viral illness during which he was able to function normally at a high level. In my case I only had about 7 weeks where I was able to function, but all the while feeling that something was very wrong.
My timeline as follows:
End of FEB 2020: Flu-like illness, after 4 days recovered
2 Days later: Sudden insomnia, feeling unwell again
Next 4-5 weeks: Lots of fatigue, malaise, other dysautonomia symptoms
Following 6-7 weeks: Partly back to office, gradual improvements, but still poor sleep and other symptoms
Then I crashed with severe insomnia, worse dysautonomia, PEM, the whole lot......
Now 2.5 years later I'm a little better, but far from recovered.

Hopefully the answers of Long Covid can help the people that have been suffering for years. We could see millions of people helped if they find out how to treat us.
The only way to be “treated” is to completely change your lifestyle and what you eat. There will never be some magic prescription. The long covid problem is systemic and complex rooted in inflammation and other chronic infections.
I have been hoping for this too.
What i find concerning is the not wanting the two groups to be considered as having the same problems. Separating the two groups is not good news. Long haulers have a real problem, it's because of the virus and the other group, still don't actually have a problem, it is all in their head.

Me too. 14 years. I knew we’d see an explosion of post viral illness cases with Covid. A novel virus being caught by so many all at once.
Being a sufferer of CFIDS I have been thinking about a pathogen or , vaccines triggering similar symptoms as long Covid. Inflammation could be the the common denominator causing other illnesses.
I had Covid (mild) March 2021, minor long Covid symptoms. After my employer pretty much forced me to get the jab in Aug, my long Covid symptoms exploded, extreme fatigue and brain fog.
My quality of life has been extremely reduced, because of the experimental drug they are calling a vaccine

Dr marsh, I can’t thank you enough for sharing your story and experience managing post viral CFS. I am 14 mos post Covid and struggling still with too many symptoms to list but the worst is the fatigue and PEM. I am working on pacing and learned so much today from your story. I’m very emotional even writing this because the validation that the PEM is real helps me so much. I’m surrounded by well-meaning friends and family that think I’m still sick because I’m not getting out enough and exercising. I've gotten better at managing it tho - planning for the PEM and saying no to activities that I know will be put me out of commission in the days following. Its a complete life changer - I use to travel 3 out of 4 weeks for work and now I struggle even working remote. I'm trying to stay positive but there are some days it feels overwhelming. I started crafting as therapy :) when the covid insomnia hits I get out my craft supplies - reserve computer for work effort - day by day. I stopped trying to explain long haul to people and just smile when they say I should get out more that exercise will help :) Thank you for asking about my health. I hope you are feeling better. @@MK-gy1ug Thank you. I appreciate the encouraging words. If you are 13 weeks post infection, the most important thing is rest. Honestly, I got COVID in Oct 2020 when there was no vaccine and no treatments. There was little to no understanding of recovery after severe infection. I was critically il for 30 days and ran a 103 fever for 12 of those days. After 6 weeks I returned to work (remote) but I was still so sick. By Christmas of that year, I was losing my hair and developed a whole new host of symptoms. I was so sick but still be scheduled to present at conferences virtually. The people I worked with were supportive but thought because it was remote work that it wasn't taxing. The brain fog really kicked in around month 3 and the fatigue was just debilitating. I kept pushing it through it but one day I fell asleep during a web meeting. Completely out. I tried to slow down but there were so many people sick with acute COVID and I felt guilty saying no to meetings. I'm a nurse - we just keep going till we fall over. My days morphed into 12 hours then 16. By the following summer, I was so sick I just couldn't do it anymore. My employer told me to take the time I needed only to have everything waiting for me when I got back. I couldn't sleep. I couldn't eat. The joint pain was debilitating. I finally made the heart wrenching decision to resign in Sept 2021. Only a year before I was named one of the top 25 female executives in healthcare IT and now I was resigning. I took 3 months off and it really helped. My hair grew back :) I gained some of the weight I had lost. I was able to sleep a little better. Since then, I've learned how to better manage the energy I have and plan for the post exertion malaise. I started a new job in january that was less demanding. That said I'm struggling even with 8 hour days. I take things day by day and hoping they will find something to help us, but I do believe that if I would have let myself recover fully in the months following my acute infection that I would not be this bad. I had no preexisting conditions. Healthy 55yo until COVID. So please MK - give yourself time to recover. Rest, hydrate, eat and clear your mind to focus on healing. God bless you all and thank you for your services.! I also wanted to ask you if the fatigue and PEM improved at all? I also have really horrific joint pain and weakness specifically in my arms. It comes and goes but it’s really horrible when I do have it. I feel like my arms are going to fall off from pain and weakness. I see some sites telling me that I should build my strength slowly by walking a little bit each day, just so I don’t sit in bed all day
And get atrophy. Then there’s other sites that say I should be resting all the time so I can avoid PEM. Not sure what the right thing to do is. I have kids which means resting can be close to impossible sometimes. But I do the best I could do!

And I needed to be reassured about the PEM as I just watched a medical expert panel online conference from a college lecturing how to help long haulers and the physical therapist greatly encouraged movement/exercise as soon as possible! Didn’t know whether to laugh or cry. And this was just a couple weeks ago.... sigh...

The reality is that only around 5 per cent of people with ME/CFS recover. Dr Marsh isn't being negative. He's simply describing the reality of living with this condition & how best to manage it until effective treatments come along.

I believe it’s like the 4 minute mile - once you believe it’s possible, that mindset will help you recover. Mindset alone isn’t enough, but not believing that recovery is likely will certainly hinder you. I know what a horrible illness this is. It really wears you down and tries to steal your hope. I’d like to send positive vibes to everyone with long covid / ME/CFS. I’m almost recovered now, 16 months after my initial illness and 5 months after getting an NLP coach to help with my mindset. And yes, gradually increase you to your triggers has now been proved to not work. Go for about 50% of your capacity so you have some margin. Pacing as mentioned is the way to go
Get a Garmin watch. It helps you read the "body Battery" and indicate stress Level. For me a game changer as it helps me to not over do things. I have alarms on heart rate and when I achieved 3000steps I make sure I stop to not get hit with post exercsion malaise!
I do this too. The visible app is releasing a polar arm band that measures HR and HR variability all day. It will apparently tell you when to rest etc. designed for long covid ME/CFs. Have you heard of it?
I have a Garmin too and it's a game changer.

Covid is tragic with the deaths it caused. For ME/ CFS and Dysautonomia sufferers it has made that tons of money is being distributed to medical studies. It's a game changer for us who have been suffering with this for a long time.

Thank you Dr Asad Khan and Dr Ben Marsh for sharing this interview. I am 14 months post covid (a very mild case) and have struggled with the exact same cycle of 'activity' (physical/emotional/mental) induced relapses as you describe (+ a number of other symptoms). It has been of comfort to hear that what I am now experiencing is most likely post viral ME/CFS with PEM. I did not have a clear understanding of PEM before seeing the interview and I have struggled to describe it to my GP. I was extremely active before all of this and my fear now is that I may never return to my 'old self'. I live in hope that there will be a treatment found soon to help us with these life altering symptoms. At least until them I can now pursue support for ME/CFS + PEM.
I take a collapsable chair for my short slow walks. I pull it out often during the walk, and sit down for a longer time than I walk. That rest is so important. I also drink more around the time I exert, so I piss out anything the cells are excreting after metabolising. As I believe the immune system is attacking these molecule. So pissing it out early I think helps. Although not enough

Andrew, sorry to hear about your CFS. I’ve actually had Fibromyalgia (FMS) and CF for the same duration. I’m only in my 8th month of the joys Long Covid. No two days are the same regarding the reactions/symptoms. Variety is said to be the “Spice of Life”, but I’m not convinced in this case! 🤗😉😉😂

Thank you for these long hauler videos!!! The ones about POTS and MCAS really helped me get on a path to feeling better. I’m not 100%, but able to function and feeling so much better. Learning about Mast Cell Activation has taught me how to tackle this. Hopefully I won’t have to be on antihistamines forever, but they have been a lifesaver. Keep doing these videos. It is really important to understand that exertion is *not* just physical.
My sister has been bedridden with Long Covid for months and we've done a lot of learning - absolutely key is that emotional stress, cognitive load and social interaction can have an even greater impact than physical movement, as these actually use a lot of energy. Even positive social interaction can have a detrimental effect on your energy, which is really hard to come to terms with...you have to trade-off, because no social interaction will of course make you more lonely which is bloody horrible!
Another crucial thing we learnt was understanding that the 'baseline' activity that doesn't cause PEM doesn't mean the level of activity that makes you symptom-free. We kept thinking that what my sister was doing couldn't be in her baseline because she was still so ill but once we realised that we should be looking out for that flare up of flu-like symptoms it became clearer. So it's about finding what you can consistently do each day without getting significantly worse 1-3 days later, but also realising that there are things outside of your control that will use up some of your energy (e.g. a heat wave, menstruation, a call with your Dr you have to do) and so when these things happen you might need to ensure you're resting more and doing less in other areas than you usually do.

Your symptoms reflect mine so closely. I have tried and thought I had found a way to manage it but not. I have 2 - 2 1/2 hours window to do my work. If I go over that I am unable to function for the next day and sometimes longer. Nobody understands this. They think I have given up or have become lazy. The harder I fight it the harder it fights back. It is now mentally destroying me. I have been a self employed engineer for many years and have carried out many demanding projects and tasks alone and love a challenge. This is a challenge that is beating me. It is destroying my life.
Given your condition iam surprised that you can talk so articulately for so long. I find my speech starts to slur and my confusion stops me completing sentences after around 45 mins. Having been unaffected by the covid infection I struggle to accept how it has now ruined my life.
As a covid long hauler since last November, I was actually quite happy when my doctor immediately diagnosed me with chronic fatigue syndrome because of my post-exertional malaise. It gave me something that I could research while they were looking for covid specific treatments. I'm lucky enough that I think I have figured out my post exertional malaise triggers. For me it is definitely heart rate and anything that requires an increase beyond standard oxygen levels into the cells. For example, I can go for a 3-hour walk if I walk slowly, but a 20 minute walk at a fast pace will cause weeks or months of PEM.
Months? How can you tell when one stops and the other starts?
@@murielh7678 I would describe it as waves stacked on top of each other. A fast walk might cause a medium wave that lasts 3 weeks, carrying a heavy object might cause a large wave that lasts 6 weeks, and trying to hold my breath would cause a small wave that lasts 2 weeks. At any given point, my current feeling was the sum total of all current in progress waves. I stopped experiencing PEM waves after about 6 months, but then went on to develop histamine intolerance which causes its own set of waves, but those are much smaller and more manageable as they just lead to mental confusion and memory issues, not complete multi-day collapses into bed. The major thing I attribute to the end of the PEM waves was figuring out that I had intestinal candida overgrowth, and going 100% gluten and sugar free, and getting stool microbiome testing through a naturopath until the candida came back negative (no candida growth observed).

Thanks for this conversation about PEM and GET. I'd love to know if any longcovid experts might have insights into taking targeted longevity supplements to address specific longcovid symptoms (such as neuropathies). I'm in the pacing and rest group, for sure, and am feeling better when shopping the imaginary longevity supplement shelves to address, for example, GI issues (butyrate & IGG), or neuropathies (alpha lipoic acid and hyaluronic acid). In addition to targeting my specific issues with supplements, I'm also feeling better now that I've backed way off on all exercise beyond daily house and garden work. I've heard some athletes with longcovid were instructed by their coaches to take 90 days off all exercise, so I'm giving that a go, and so far feeling the best I've felt in the last 16 months of my longcovid journey.

I've been taking Glutathione and Quercetin and Zinc as well as many others... the recent add-ons are those that are giving me noticeable huge improvement to my GI and neuropathy issues that seemed to hang around and worsen on relapse. I haven't worked with the other aspects you mention, but I'm so relieved to see such positive improvements in my GI/gut issues by simply adding to the usual MCAS and NAD+ stack with a few personalized supplements. I'm still not recovered from longcovid (still can't exercise as before), so for that I'm backing further off all activities. I am learning that left to my own devices, I tend to be an overachiever, and that is not helpful when it comes to pacing and rest. I've messed around with numerous supplements. Mostly unsuccessfully. The notable exception was gut bacteria. It didn't really make me feel any different but my poo became way better and normal again. So it kind of helped alleviate just one symptom. However none of it really made me feel much better in terms of energy, concentration etc. Just better poo.

I have had cfs for 20+ years. NOt even diagnosed officially, let alone helped or supported by my country's equiv of NHS.. I was looked at by some Dr's as a hypochondriac, 'hysterical', 'just' depressed late 30's/early 40's woman. I used to be a Reg Nurse. My hubs kept telling me until a Dr gets CFS ( or now Long Covid) they will not care abt it .My hubs had open heart surgery last year and within a few weeks of the op, had more energy than I did. he was back at work after a few months. I have not been able to work for over 10 years.
Excellent video again! I relate to Dr Marsh as I too am housebound 99%. The point I would make is that GPs are still in the dark with ME CFS and I think they choose that. Any help I have received has been at personal expense, GPs simply are not interested!
I think treatments for Long covid may come along, but those with ME CFS will not receive or benefit. I've battled just to get antihistamines prescribed and paid a private doctor. The NHS have only one concern, minimising costs.
Anyone who has tried pacing will know why psychological support is so valuable. It's really difficult to do.
I will get 2/3s into a 'normal day' and it will start. I will feel it behind my eyes first. If I don't lie down and sleep, I feel like I'm going to die. Sometimes I will wake up wondering what day it is. It is often like 12-14 hours later! For much of the pandemic, all I did was work (while feeling badly) and sleep. It feels like I have lost 3 years of my life. Ironically I had to work harder often, because I had to teach both online and in classes. But I had nothing for teaching online. I would spend many hours creating materials for online. All I did was work and sleep. I don't remember enjoying much of life at all.
Really good series Gez, guess I’m not the only one who took up Duolingo and learning Chess online then, and even got a slack line with my kids to begin some form of exercise. Still got the chronic fatigue, brain fog and exercise intolerance but the recurring bouts of flu have subsided somewhat when I have a relapse. Been referred to the CFS/ME clinic, overlap of symptoms are very close, just wonder that the pathogenesis of long Covid is maybe different for us.

The survey mentioned was a questionnaire done by the ME Association, given to people with severe ME, about their experiences of treatment.
You point out that the draft guidelines were supposed to be finalised in April, but that this ‘has been delayed’. I would think it likely that the powerful psychiatric lobby in the UK are making a last ditch effort to have CBT and GET reinstated. There is the threat of reputational damage and possible even of legal action taken against them, especially if it were shown that they knew of the harms and still prescribed GET. They will be doing everything they can to block these proposed changes to the Guideline.

I'd be curious to know if the community has any experience in the following: For me (along with the usual symptoms described here) it's as though my circadian rhythm is now broken. If I change my productive period to 1am to 7am and sleep till 3pm, I have so many better, productive, almost normal "days", compared to trying to be awake and functioning at the same time as the rest of my time zone and suffering the debilitating effects of this.

This has definitely been my experience too. To get myself back into a regular sleeping schedule I've been taking melatonin, magnesium, gaba, and Valerian root. I need to take about five valerian root pills with that combination for it to work.

I like others have tried to describe my symptoms to a my cardiologist and to the nurse practitioner of a covid cardiology specialist no one seems to be listening I had post covid pericarditis was prescribed beta blockers but the made my tachycardia go from tachycardia to bradycardia to say drop my pulse to 43 which made me lightheaded and feel like I was going to pass out. Now they want to try calcium Chanel blockers i dont know whether to try this or not. They want me to do the exercise program I walk at least 5 miles a day as a nurse the fatigue is very disabiliting the nurse practicer said this exercising of walking at work does not count
Exercise programme sounds like a terrible idea.
I read that this virus appears to thrive (and replicate) with exercise-induced oxidative stress, is this true? If so, should you not exercise. I get out of breath very easily during exercise but feel fine when I finish.
I would avoid strenuous activity till your symptoms have resolved for at least a few weeks

@@Beekind799 hate to say it but you're actually on to something here as I had a urine chemistry test just as I was getting sick last year and it showed EXACTLY this. Thankfully I also know atp methylation mechanisms and barely get fatigue with my thankfully now nearly resolved residual lh, especially since also treating with a course of Ivermectin and daily full mcas protocol medications and supplements.

Any luck with higher Magnesium intake? or any other dietary substances involved in energy metabolism? CoQ10, B vitamins?
I am taking Nadh, coenzyme q10, b50 complex, melatonin, magnesium, zinc, selenium, quercetin, cod liver oil, omegas, and some other stuff. It's all helped with my baseline energy and mental state, but nothing is preventing post exertional malaise at this point.

I have been struggling with all this 5ys but being female,nurse, over 50 was laughed at then muscle biopsy said IBM, which doesn't have these sx. I pressed for mestinon despite being double sero neg. In one day eye pain, diff focusing, pain from muscles collapsing, difficulty breathing all eased. Neuro still not convince can have 2 rare diseases but keeps writing mestion which has more than doubled single breath count. I did have covid last yr in March when all was chaos and luckily treated myself at home with ordered bipap and sl inc FiO2 to stay above 91. 5 mos later could get dressed and go out if someone else drove and used neck pillow, cane. I am just wondering if Mestinon might... did see it is used sometimes in dysautonomia which surprised me. Neuro isn't upset my HR goes from 49 to 104 for no apparent reason, or that mental stress can wipe me out as well as walking and a cell phone is too heavy at times.

I have a question - with POTs the recommendations seem to suggest that recumbent movement is helpful to start and then a slow gentle progression to more upright movement and exercise will ultimately help reduce symptoms but........when POTs is a secondary dysfunction like in long covid or other viral illnesses it seems that exercise can be damaging so.......when like me and many others I have POTs and clearly post viral exhaustion from long covid it seems to me that the deconditioning due to having to rest loads to help with the long covid is not helping the POTs at all - would welcome any thoughts but is this maybe why the POTs is not improving in those of us stuck in this position I wonder?
Hi Patricia - the hope would be you can do recumbent exercises that keep you inside your energy envelope and don’t induce PEM.

It seems a problem for some that this diagnosis is 100% excluded if it starts in childhood (or even adulthood) and without diagnosis because it is no longer possible to "never occurred" previously.

I'm finding the fatigue incredibly debilitating. I'm six months in with long covid . Are you all seeing improvements now in your fatigue ?
It's the pacing that's difficult . I went for a walk Monday and I felt good and it didn't over exert me but then by Wednesday , all my symptoms flared up with chills , aches , fatigue and IBS . Will the systemic inflammation eventually go away ?
I thought I was in tune with my body but clearly I pushed it too far . I've relapsed for days now.
Hi Ella, the trick is not to push it too hard on the good days. You’ll feel fine at the time but it will bite you later. 6 months in was also the worst period for me so part of it is also the course of the illness. Be patient! :)

I’ve had the same symptoms since my second Pfizer . Glad to learn more about it. Poor Dr Marsh … you could see him getting fatigued and brain fog about half way through.
I have Long Covid and really not well, had a really bad week and my daughter ended up taking me to the hospital....the doctor told me I didn’t look ill and he had had covid and he had recovered, and if I wanted to see poorly people he could take me to a couple of wards....

Can post viral fatigue linger post six months but not be defined as CFS/ ME? . I am trying to hold onto the thought that I will recover from these symptoms. I've read that only around 13% of people recover from CFS hence why I'm really hoping my body will recover from this illness . I'm almost six months in though and the fatigue is still persisting as are the mental /physical exertion issues
Yes, chronic fatigue syndrome is a terrible name. The illness is a post viral disease which includes dysautonomia, POTS, NMH, other forms. Brain fog is another symptom.
Post exertional malaise which is mental, emotional and physical exertion. Practitioners still encourage exercise which is known already to cause further damage. There is Hope . Joining together to learn is important , there is much proven evidence available.

My experience of ME is to sleep as and when you need to. If you cannot sleep, meditate and do deep body relaxation instead. It may help during the day to try and keep more to deep body and mind relaxation if you can as that may help you sleep at night. If you have to be up at a certain time, allow an hour of deep relaxation and diaphragmatic breathing beforehand and drink two large glasses of cold water to help with orthostatic intolerance. One of these preferably being a rehydration drink. During the day it may also help to lie down frequently with pillow under knees to secure blood flow and oxygen supply to brain. I combine this with relaxation exercise and slow diaphragmatic breathing for emergency recovery
