It’s better to apply with usmle scores, however yes you can update that later.
Can I submit the application before I get the OET exam and ECFMG certification?
You can, but it is better to submit it with ecfmg certification
what if someone takes step 2 on sept 15 , is there any way they can put their foot into this match cycle?
Can you apply? The answer is year! However some programs who filter applications earlier may not review your application

You have to rotate with attending to get LOR

They can be both paid and unpaid. Usually 1 letter is granted for 4 weeks of rotation.

It is still valid as long as you are able to upload to your application.
