There doesn't appear to be any guidance stating this. The fact that you can also use a money order makes it seem as if the check owner's name is irrelevant.

For immigration purposes, I want to request my spouse's tax returns to my address and the address on her file is her mother's. And she doesn't want to change/update that address (form 8822). So, should she write my address in line 3 or 5 to request the tax return at my address? She is a single filler as of now.
Do you mean that your spouse is filing a separate return (if you're married, she can't file as a single filer). If she is sending the tax return to you, then you would be considered the third party. Your spouse would complete Form 4506 (and pay the user fee), and complete Line 3 with her address information, then Line 5 with your name and your address information.

This is the form that you would file to receive actual copies of your tax return. If you're looking for transcript copies, then you should file IRS Form 4506-T. According to the form instructions, it may take up to 75 calendar days for the IRS to process your request. There's not really a way to track it, that I know of. However, you can also get tax information by setting up and checking your online IRS account. That might be a faster way of obtaining specific tax information.
I just checked the updated form, and it appears to be $30 per requested return.
