As a manly man, who mans about everyday I do not know how pads are, but I definatly know that, as long as you do not wear see through pants, leggings or hot pants no one would even guess a diaper under clothes. I have even worn shortalls with a diaper under them in public and no one even blinked at me. Tbh, I have a little insight and also a little's insight. Ofc it is a coping mechanism, in my case it helps me with my depression and with actually being a functional adult, as contradicting as it sounds, cause without my teddybear in my backpack I would not be able to go out to work. But, for intrinsic ab's, there allways is a sexual component. Freuds theorie of infantile sexuality is one way of explaining this. This component is there, but, and that is true in my case, but generally more true for women, it can be very small. So much so, that it is not even a part of the behaviour, for many AB's.
But it is not a responsible way to deal with them, they need to get real help

I actually had an ex boyfriend who went through a lot of emotional and physical trauma and abuse (we had this in common, though I have different coping methods) and his coping method was to enter "little space" this was not anything sexual or kinky, it was like his mind would actually shut down and reboot as a small child. It was like a light switch, he'd start acting like a small child when he felt overwhelmed and wouldn't come out of it until he'd slept a bit. He had intense social anxiety and this would often happen in public spaces with lots of people, and I would sometimes have to lead him to a safe space.
