Excellent talk as usual sir! The only comment I have is that one can actually wire the back end of the guideliner with a buddy wire while the guidliner is inside the guide, so you dont really have to take your guide extension out when adding a buddy wire. Thank you so much!

Thank you! and thank you for the comment. I agree with you. There is a risk of damage of the wire tip while it is going through the monorail collar of the Guideliner, particularly with stainless steel nonpolymer wires. This was a bigger issue with the first generation Guideliner, which had a sharp metallic collar at the monorail junction. The transition is now softer and smoother. It is still my preference to take the Guideliner out then buddy wire, depending on the case.

Dear Dr Hanna. Concerning support with the guide. Sometimes while advancing the material it backs up the guiding catheter. What are your tips and tricks to reposition the guiding catheter for better support. Thank you

I have written a general cardiology textbook, which has some cath and angiography chapters (Practical Cardiovascular Medicine, Wiley). I have not written a specific interventional book.
