One thing I always find challenging in my large CE implementations is "How to find out that flow that just broke", I find myself into situations where I've to debug and fix a bug that involves fixing the power automat which was written by somebody and that person is no longer in the team, so, how do we find out a flow which just broke and then go on editing it and fix it!!!
