Thank you Dr. for the comprehensive explanation of an elevated PSA and prostate cancer. I have BPH and an elevated PSA. I am scheduled for prostate cancer biopsy April 15. Your lecture calm my fears and give me greater strength to face my medical predicament. Many thanks again Dr.
I found out I have Prostate Cancer last year. I got the MRI which showed 2 small spots. I got the biopsy done and it showed I have a Gleason score of 3+ 4. I then got a Decipher Genomic Test done which showed it is a low grade slow moving Prostate Cancer. I then went to discuss it with the specialist I chose to do Cryotherapy if I needed any treatment. He advised me to definitely do active surveillance at this time based on all the medical documents. He said the treatment would hurt me more than the cancer at this time. I took his advice and chose to do active surveillance . I will get my PSA checked every 4-6 months to make sure it doesn’t rise above the normal score.
@@Delani11 I was 5.1 when I got my MRI. I’m going for another MRI in the next few weeks. My last PSA was 9,8 which didn’t make sense to me but your PSA can bounce around for different reasons but the MRI will show if there is any change in the size of the 2 spots on my prostate. I’m hoping there were no changes.
Humm🤔 Real confusing right? I'm 66. My PSA was 2.5 a year ago, 2.7 6 months ago, and now 3.9. Got MRI done after my first PSA of 2.5. Shows 3 lesions. One at 1.6 cm, one at 1 cm, and 1 at 13 mm. Radiologist says very suspicious. No biopsies, as of yet. Trying to avoid an unnecessary biopsy. Pi-rads 5/5, 4/5, & 4/5🤔 But, prostate size is 56 cc. So, my prostate density comes out to be 0.04. Which is considered, unlikely to be high grade disease, according to the research. This is the equation - PSA 2.5÷56 cc volume = 0.04 density. Low risk supposedly. Just got another MRI done, 2 days ago. Hopefully a better outcome, or at least the same. Hopefully a different radiologist reviews my MRI, get a different perspective from the first person.
@@AnthonyShaw-ty9pi you have lesions and they didn’t biopsy you yet ? I would get the biopsy and check your Gleason score . If it is 3+3 you should be ok but if it is 3+4 you should tell your doctor to send it to Decipher or whatever genomic test company they use for piece of mind. My genomic test came back favorable as low grade and the chances of it spreading in the next 10 years was about 1.87 percent. Your doctor did not suggest a biopsy with 3 lesions ?
@@shamrock8561 Did your doctor do extended PSA testing? The total PSA, free PSA, The PSA density and the velocity-- PSA health panel. and order the 4K score test, and did you do the urine test?
Lol , “don’t panic”, until I give you all my reasons to make you panic! Since PSA was introduced and went global about 1980 around 3,000,000 (yes three million) men have been overserviced, wrongly been relieved of their prostate gland. Don’t ever be told that prostate has only ONE purpose.
Everyone should be their own advocate when it comes to their health. Don't rush into a procedure. Research your options, and find a specialist you trust, and get second opinions. there are many non-invasive modern testing. Do extended PSA panel testing, 4K, urine MRI etc. Medicine is not 100 percent Surgeons are going to push surgery and so on. Also research the doctors you see. Your want someone with many years of experience, even if you need to seek them out of your local area or state.
Yes, up until 2005 they were recommending treatment for Gleason 1s and 2s. So many men had prostate treatment for something they don't even report on pathology reports now, as Gleason 1 and 2 aren't considered "cancer" any more. I have Gleason 3+3, which I am reluctant to treat, since there seems to be debate in the Uro community as to whether Gleason 3 is "cancer" and whether it spreads. Since I don't want to be one of the last patients treated for Gleason 3, and then a year later they decide Gleason 3 isn't "cancer" anymore, I'm on AS until they show me a higher grade "cancer". Many, many men have been over treated for prostate cancer that wasn't really cancer. I don't want to be one of them. If it wasn't for the more than likely, life damaging side effects of treatment, it wouldn't be as much of an issue.
@@scottgraves4754 Great analysis! These posts are giving me hope! Thank you so much for sharing your thoughts and experiences. Actually a 3 sometimes some people are now saying isn't considered cancer 3 months after you made your post! WOW!
