Well... when you open a bag of store bought flour (white, bleached * gasp * , enriched) -- it doesn't smell like much. Now... freshly ground?! Oh lovely. :-) I've got a small electric grain mill at home and like to show people the difference. Their reaction is usually much like hers. ;-)
The gears are engaged when you release the brake to allow the water wheel to turn. So, you are inside the mill when the machinery is set going. All you see outside is a turning wheel.

The few water-powered mills I am familiar with either had the wheel set into a "race', where the water pushed the wheel from underneath, or the water flows from above over the wheel, turning it. The external overhead view of this mill showed a still pond. I didn't see a race. What is turning the wheel, and how?
The great majority of humans can't invent. It takes time for the right person to make the discovery. And it is the discovery that takes time, regardless of how complex it is.
As I commented elsewhere already, she's probably only used to store bought white flour; bleached and "enriched". When I first started using my grain mill, I was amazed at the lovely smell, too- and the flavor. :-) Oh, and one could mention the health benefits! ;-)
