Milk fever, also known as hypocalcemia, or parturient paresis, is by no means a new condition for modern dairy cattle. It is one of the most common bovine metabolic disorders resulting from calcium deficiency. Milk fever affects all cows in the herd approaching calving or just after calving to some degree even though we do not see it.
Calcium demand for a cow starting lactation is almost double compared when she was not lactating and pregnant. Normal blood plasma Ca concentration is tightly regulated and generally kept between 8.5 to 10 mg/dl (2.1 to 2.5 mM; Cahn and Line, 2005). Two to three days before calving, a high amount of calcium (about 11g/d) is moved from the mother to the fast-growing fetus and to the mammary gland for colostrum and milk production (about 23g/d) shortly after (Goff and Horst, 1997a).
Rapidly increasing demand for calcium is difficult to cover (calcium level in colostrum is 8-10x higher than in blood).
Around calving, blood Ca level may drop below the normal range leading to homeostatic failure. The point at which it is considered clinical was suggested to be below 5.5 mg/dl (Goff, 2008) but reaches a subclinical stage when it drops below 8.5 mg/dl.
