For me, it's the setting. You find yourself out alone in the forest, being silently stalked. Your stalker is an enigmatic vaguely human beast (whether it feigns a man or was a man now corrupted into this form is unknown) that gets more aggressive as the fight goes on. Nobody's around to save you, just the stars that will watch you die.
It's a reference it rounds up damage and replaces other items with itself the more you use it the more it's effects lessen it's like spice from enter the gungeon
I mean the far shore bosses are pretty much all just 'guys' so it fits pretty well. Yea he's a bit goofy but not any more goofy than some of your own party members like Anton or Arnold

A "doula" is basically the psychiatric version of a midwife. Hence the fetus the Doula boss is holding.

There's something so viscerally disturbing about Doula. I don't know why, but something about it seems so very wrong.

@HansVinlin the theme with all area 1 bosses is that they are human (or were) so its really the unicorn that's out of place
