There are a few variants from here, but essentially Black is threatening to queen the c pawn with check and either: (a) force mate by weight of material (if White avoids the queen exchange); or (b) swap queens and then win White's advanced h pawn after … Nxf4 and then … Ng6, blocking the promotion on h8. It's worth playing through this long ending several times. One of the most beautiful "combinations" I've ever seen, starting with the long king walk from home base.
