Justin Long was hit just above his eye by the wrench and bled from the cut it made. Him yelling out in pain and dropping to the floor was a genuine reaction. The wrenches were made of rubber so they wouldn’t be as dangerous as real ones, but poor Justin was not so lucky.
Rip Torn did a fight scene once where famously bashed his director and co-star, Norman Mailer, in the head with a hammer for real. Norman bit Rip's ear. They continued to fight until the cast and crew broke them up.

Why all the blurring of of parts of the clips? yes there is violance in the clip, that is the point why this clip is put up, but do not show the clip completly.. sooo I stopped watching it.. sad to see all the blurring these days...
They do it for TH-cam. If too much is shown in clips, the content creator gets a strike. That's why whenever TH-cam content creators do reactions, they don't show the whole movie.
It's likely unrelated to the violence in the scene. A lot of reactors periodically blur movies/shows to prevent copyright strikes. Some are more cautious than others. Compare the blurring in the first pair of reactors to that of Addie Counts (like 5:40 or so into the video).
