No need to attach PAN card with form 15G. But make sure your PAN number is linked with your EPF account, otherwise form 15G is not valid and 34% tax will deduct.

Field 16 lo Mottham employee + company amount with interest veyali. Pension amount akkarledhu, only PF amount ke form 15G. Field 17 lo ee PF amount ki mee approximate estimated annual salary kalipi mention cheyali.

5 years means including all the companies in which you worked. But PF should be transferred to your last company.

You don't need to consider pension for form 15G, it is required only for PF amount.
